Two patients developed cavernous sinus thrombophlebitis (CST) from tooth infection.A 36 year old male had severe headache with bilateral 3rd and 6th cranial nerve (CN) palsies after extraction of his left upper 3rd molar.Another 53 year old diabetic male had fever, headache and bilateral complete ophthalmoplegia after a tooth infection.MRI of brain in both patients showed bilateral cavernous sinus abnormal enhancement with partial thrombosis.Broad spectrum antibiotic plus low molecular weight heparin (LMWH) successfully resolved all symptoms.Both fully recovered without any recurrence in follow-up by 3 months.Although rare, CST causes high mortality and disability.Timely diagnosis and treatment is critical to full recovery.
