MRI模拟定位在鼻咽癌肿瘤靶区勾画中作用的初步结果

探讨MRI模拟定位在鼻咽癌肿瘤靶区勾画中的意义.对2015年1月~2015年9月北京大学第三医院初治的病理诊断明确鼻咽癌患者14例,T分期按98鼻咽癌分期标准:T1期1例;T2期2例,T3期8例,T4期3例.由3位放疗科医师(高年资主治医生或副主任医师以上职称)分别对每例患者增强CT图像及MR1图像进行盲态独立勾画靶区.为方便比较,仅对所勾画鼻咽部原发病灶的靶区(GTV)进行统计分析.每个病例分别拥有3个基于CT图像的GTV(以下简称为CT靶区)及3个基于MRI图像的GTV(以下简称为MRI靶区).对CT靶区及MRI靶区,比较基于CT、MRI靶区的体积、不同医生所划靶区的重叠度、变异系数.统计方法:配对样本t检验.MRI在显示软组织方面具有明显的优势,尤其用于分辩肿瘤组织与正常肌肉组织、良性病变的鼻甲赫膜,判别颅底、斜坡、翼突等是否受累等。CT能较好地显示颅底骨皮质破坏,且对圆孔、卵圆孔、颈动脉孔和颈静脉孔的破坏显示较为满意。本研究MRI及CT两组勾画的平均靶区体积未见明显统计学差异,须后续工作中增加研究病例。
