Cryptogenic organizing pneumonia (COP), a type of organizing pneumonia without the presence of evident pathogens, such as infection, or other associated diseases, such as connective tissue disease, is a relatively rare extraintestinal manifestation of Crohn's disease (CD).Moreover, it is easy for the patient who is under anti-TNFα therapy to develop some opportunistic infections such as cryptococosis, especially in older patients. In this case we described a patient with CD who developed cryptogenic organizing pneumonia and asymptomatic pulmonary cryptococosis simultaneously and relevant literatures were reviewed. He was diagnosed by percutaneous lung biopsy and treated with prednisone and fluconazole. And a follow-up CT scan 2 months after treatment showed almost complete resolution in the lung.
