Cryptogenic organizing pneumonia (COP), a type of organizing pneumonia without the presence of evident pathogens, such as infection, or other associated diseases, such as connective tissue disease, is a relatively rare extraintestinal manifestation of Crohn's disease (CD).Moreover, it is easy for the patient who is under anti-TNFα therapy to develop some opportunistic infections such as cryptococosis, especially in older patients.In this case we described a patient with CD who developed cryptogenic organizing pneumonia and asymptomatic pulmonary cryptococosis simultaneously and relevant literatures were reviewed.He was diagnosed by percutaneous lung biopsy and treated with prednisone and fluconazole.And a follow-up CT scan 2 months after treatment showed almost complete resolution in the lung.
