Because of heavy atherosclerotic plaque burden, inability to see the course of the vessel at the site of occlusion, and often longer length of lesions, reliably crossing a chronic total occlusion (CTO) with a coronary guidewire is difficult and is the most common reason for failure of percutaneous coronary intervention (PCI) for CTO.
