Medial patellofemoral ligament (MPFL) reconstruction is a viable option for correcting lateral patellofemoral instability.Many of the complications that can arise from MPFL reconstruction are the result of technical error.A revision of postoperative failure of the right MPFL reconstruction was reported in a case.In addition, the purpose of this report was to discuss the multiple failing factors , including no need of concurrent realignment surgery, the positioning errors involved tunnels, improper graft isometry, and iatrogenic lesion.
