As the use of CT Scan and MRI continues raising an increasing number of adrenal nodules are being diagnosed.The work up of these nodules aims to detect functionality and risks factors for malignancy.The posterior retroperitoneal laparoscopic adrenalectomy (PLA) is a new approach that can be used in the management of adrenal lesions with indications for surgery.In recent years PLA has become the preferred approach for resection of most adrenal nodules.PLA requires the surgeon to be familiarized with the anatomy from a retroperitoneal perspective and the surgeon should get used to operate in a rather reduced anatomical space.We studied the patients who underwent PLA at University of Puerto Rico to evaluate the outcomes in terms of learning curve, operative time, rate of conversion to open adrenalectomy, estimated blood loss, hospital stay and complications.In addition, we also took a look at post-operative blood pressure parameters in patients who had functional adrenal tumors.We also compared our series with other published series of patients undergoing PLA around the world.We found that the learning curve to be skilled in this procedure is short and once there, PLA becomes an ideal surgical approach for patients with adrenal nodules.
