Background: Chronic cholecystitis is a common inflammatory disease of the gallbladder.It is related with various gastrointestinal tumorigenesis, although the pathogenesis is not clear.The objective of the study was to investigate the impact of chronic cholecystitis on survival time in patients with advanced colorectal cancer through more than 5 years follow-up.Methods: Cases that confirmed colorectal cancer pathologically in Nanfang Hospital i between 1.1.2000 and 31.12.2007 were enrolled in this study.507 cases of advanced colorectal cancer in stage Ⅱ, Ⅲ,ⅣV were included in the study while stage Ⅰ cases were excluded from the study.All the cases were followed up until August 30, 2012.5 patients lost follow because the death time was not clear.The rest of the 502 patients living status and survival time were evaluated.Stratified COX proportional hazard regressions were used to estimate the adjusted hazard of chronic cholecystitis, surgery, chemotherapy, TNM staging of patients with colorectal cancer or other factors to the patients survival status and survival-time.Findings (1)The chronic cholecystitis group have a shorter survival time than non-cholecystitis group(p =0.007, RR =0.631, 95%CI=0.453~0.879) ; (2)Surgery, chemotherapy could prolong survival-time in patients with colorectal cancer and reduce their mortality(surgery : p =0.015, RR =1.637, 95%CI =1.100~2.434; chemotherapy: p =0.004, RR =1.773,95%CI =1.203~2614) ; Higher the TNM staging was, shorter the survival time was.(TNM staging p<0.001,RR=3.842, 95%CI=3.150~4.687) ;(3)There were no significant differeces in patients age, gender, gallbladder polyps, cholecystolithiasis and history of cholecystectomy on CRC patients survival time.(p > 0.05).Conclusions:Chronic cholecystitis significantly reduced the survival-time of the patients with advanced colorectal cancer compared to patients without chronic cholecystitis.(median survival time in cholecystitis group: M=45.03 months, median survival-time in non-cholecystitis group :M=67.40 months, p < 0.001, z =-3.192).
