Introduction: Inordertoevaluatemuscularfatdegeneration,wehaveutilizedafatfractionassessmenttool(PACS)usingMRI quantifyingmuscularfatcontent. Weproposedusingthisnewmethodologytoevaluatethecorrelationofrotatorcufffattydegenerationandtendon retraction. Wehypothesizedthattherewouldbepositivecorrelationsbetweenincreasingfatfractionvaluesandworseningtendon retraction,andpatientage. Methods: TwohundredandeightcasesincludedshoulderMRIwithoutcontrast. Sequencesobtainedoblique-sagittalT2togeneratefatfractionmapsusingthePACSsystem. Fat-fractionvaluesofSSTmusclewereobtainedthroughtheuseofcustomsoftware. TherelationshipbetweenfattydegenerationofmuscleandpatientagewereassessedbyPACSSystem.And,the relationshipbetweentendonretractionandpatientagewerecorrelatedtothedegreeoffattydegenerationassessed. Results: Intra-observerreproducibilitywas0.92,andinter-observerreliabilitywas0.844. Therewaspositivecorrelationsbetweenincreasingfatfractionvaluesandpatientage. Especiallymorethan70s,fatfractionvalueincreasedabruptly. Themeanfatfractionwassignificantlyhigherinthesettingofamassiverotatorcufftearascomparedtonotear,small tears,mediumtearorlargesizedtears. Themeanfatfractionofmuscleswithretractedtendonwassignificantlyhigherthanthefatfractioninthesettingofno retractionfortheSST. FatfractioncorrelatedwithGoutallierclassificationinSST. Conclusion: ThefatfractionquantificationusingPACSsystemallowsforpreciseandreproduciblemeasurementoffatty degenerationoftheSSTmuscles,andthesemeasurementsarecorrelatedwithtendonretractionandage.
