Objective To explore the effects of modified circumcision on the degree of satisfaction of the patients using multiple-factor analysis.Methods Five hundred and eight patients that received circumcision in 5 hospitals in Zhejiang Province were included in the present study for the epidemiological study.Another 942 patients were selected and randomly grouped into 3 groups,namely conventional circumcision group,Shang Ring group,and modified circumcision group,with 314 patients in each group.Compound lidocaine cream(5%)was used for topical anesthesia before the circumcision for the patients in modified circumcision group,while 2% of lidocaine was injected for dorsal penile nerve block for patients in conventional circumcision and Shang Ring group.Manual saturation and Chinese Shang Ring circumcisio.n technique was used in conventional circumcision and Shang Ring group,respectively.Postoperative complications,pain score,intraoperative blood loss,operation time,incision healing time,satisfaction with appearance,and overall satisfaction rate were compared among the three groups.Results Significant differences in favor of Shang Ring group(P<0.001)and modified circumcision group(P<0.05)were found in operation time,intraoperative blood loss,and satisfaction with appearance as comparing with conventional circumcision group; however,no significant difference was found between Shang Ring group and modified circumcision group(P>0.05).Significant differences were also found in favor of modified circumcision group in incision healing time(P<0.01),intraoperative pain(P<0.001),and overall satisfaction rate(P<0.05)while comparing with conventional circumcision group; however,no significant difference was found between Shang Ring group and conventional circumcision group(P>0.05).In contrast,significant difference in favor of conventional(P<0.001)and modified(P<0.001)circumcision groups was found in postoperative pain as comparing with Shang Ring group,while no significant difference was found between conventional and modified circumcision groups(P>0.05).Except for hematoma which was not found in Shang Ring group,the incidences of other complications were the lowest in the modified circumcision group than in conventional circumcision group and Shang Ring group.Conclusion Using topical anesthetic and modified circumcision are recommendable as they can significantly reduce the operation time,intraoperative blood loss,incision healing time,postoperative complications,and pain,and increase the satisfaction rate of the patients.
