Changes of Serum and Urine Ngal in Type 2 Diabetic Patients with Early Nephropathy before and after

  Objective: To explore whether neutrophil gelatinase-associated lipocalin (NGAL) could be used to predict the progression of diabetic nephropathy in type 2 diabetic patients, and whether telmisartan can help to explore the main cause of NGAL change.Method:74 type 2 diabetic patients were divided into three groups according to their 24 h-urinary albumin excreting rate.A prospective controlled trial that treated with telmisartan for a random time lasting for 3 or 12 months can help to explore the main cause of NGAL change.68 patients accomplished in a second prospective controlled trial, and 43 of them were treated with telmisartan, 25 were used as control.Urine NGAL (uNGAL) and Serum NGAL (sNGAL) were quantified by ELISA.Results: An increased tendency of uNGAL and a decreased of sNGAL were detected, from normoalbuminuria group to macro-albuminuria group.sNGAL rise after follow-up.There is a strong correlation between sNGAL and uNGAL.The correlation differed in different period of DN and changed after telmisartan treament.Conclusion: sNGAL may be more useful in early detection and uNGAL may be more meaningful in renal function assessment.The contraction of sNGAL might be relating to the rise of eGFR in type 2 diabetic patients with microalbuminuric nephropathy.Telmisartan plays an important role in improving glomerular filtration and tubular reabsorption.
