[研究背景和目的]调强放射治疗(Intensity Modulated Radiation Therapy,IMRT)是肿瘤放疗史上的一次变革,其可以在最大限度杀灭肿瘤的同时保护周围正常组织,提高放疗的增益比,准确的靶区和危及器官(Organ at risk,OAR)勾画是发挥IMRT优势的前提。目前,头颈部OARs勾画差异大,不同肿瘤中心间勾画范围不同,由此可导致较大的剂量体积参数的差异,除此以外,OAR勾画差异阻碍了正常组织剂量体积参数与晚期损伤的研究及统一的正常组织并发症概率(Normal tissue complication probability,NTCP)模型的建立。头颈部OAR勾画指南和图谱统一了正常组织勾画范围,减少主观差异;基于图谱的自动勾画软件(Atlas based auto segmentation,ABAS)可减少客观勾画差异,但也质疑这种体积差异的减少能否转换成剂量体积参数(Dose volume parameter,DVP)一致性地提高。
