Minimally invasive surgery in the field of gynecologic oncology has progressed much over the last two decades.Conventional laparoscopy was introduced in the 1990s as an alternative route to traditional open cancer surgery.It had the benefits of reduced blood loss, smaller skin incision, less wound pain, faster recovery and less adhesion formation.However because of the fear of inadequate radicality of the surgery, increased costs, potential impact on survival and the lack of advanced laparoscopic training, there was no widespread use of this modality throughout the world except in top cancer centres.However, clinical evidence over the last decade proved that laparoscopic cancer surgery is feasible and safe in the management of early cervical, endometrial and even ovarian cancer.The development of robotic technology has facilitated the application of minimally invasive surgery in gynecologic oncology since 2005.In fact it is designed to improve the surgeons laparoscopic skills and remove the limitations of conventional laparoscopy.In this lecture, I will examine the current evidence on the role of laparoscopy and robotic assisted surgery in the management of cervical, endometrial and ovarian cancer.
