This study offers an objective appraisal of available evidence regarding the threshold for frozen section tests in diagnosing periprosthetic infection. Pooled summary estimate for sensitivity, specificity, positive likelihood ratio, negative likelihood ratio and diagnostic odds ratio (OR) were compared between ten and five polymorphonuclear leukocytes per highpower field as the threshold. The total cohort included 1011 patients and the infective rate was 19.2%. The results showed ten polymorphonuclear leukocytes per highpower field would achieve higher specificity than five polymorphonuclear leukocytes(p=0.007). And such threshold would not statistically decrease the sensitivity and diagnostic OR. In sum, Ten polymorphonuclear leukocytes as the threshold makes better performance in diagnosing periprosthetic infections.
