在诊断颞下颌关节疾患中应用 MRI,是因为它能直接描述关节盘的情况,又能非常容易地得到矢状断面的图像。其断面角度的设定,不受患者体位的影响,可以任意选择。另外既无损伤,又没有 X 线的照射。80年代根据颞下颌关节盘前移位和活动度异常提出了颞下颌关节功能障碍的概念。但关节盘位置的异常,至今在临床上仍采用关节腔内造影来确诊。然而,必须进行关节腔穿刺。因此,希望有一种对关节盘的位置和活动状态无损害的检查方法。MRI 检查法正是这样一种检查法。X 线 CT 也是一种非伤害性的关节盘检查法。但是,如果要直接进行矢状面摄影,患者要摆一个很困难的强迫体位。即便能了解到关节盘位置的情况,也不能明确描绘出关节盘的形态。所以,临床使用上受到限制。而利用 MRI 描绘颞下颌关节盘的位置和形态,则能得到相当清晰的图像。
