目的:探讨子痫前期孕妇血浆、脐血及胎盘组织中晚期氧化蛋白产物(AOPP)及内皮素1(ET1)表达水平及临床意义。方法:2012年1月~2014年1月选取辽宁医学院附属第一医院妇产科收治的80例子痫前期孕妇为研究对象,其中轻度子痫前期38例,重度子痫前期42例,另选取同期收治的40例正常产妇为对照组,采用分光光度法测定两组血浆、脐血中AOPP水平,采用ELISA法测定两组血浆、脐血中ET1水平。结果:轻度、重度子痫前期组患者血浆、脐血中AOPP、ET1水平均高于对照组(P<0.05),且重度子痫前期组患者血浆、脐血中AOPP、ET1水平高于轻度子痫前期组(P<0.05)。轻度、重度子痫前期组患者胎盘组织中AOPPmRNA、ET1mRNA水平均高于对照组(P<0.05),且重度子痫前期组患者胎盘中AOPPmRNA、ET1mRNA水平高于轻度子痫前期组(P<0.05)。AOPP、ET1联合检测子痫前期的准确性、灵敏性、特异性高于AOPP或ET1单一检测结果(P<0.05)。结论:子痫前期发病与孕妇血浆、脐血及胎盘组织中AOPP、ET1高表达水平有关。通过联合检测子痫前期AOPP与ET1能有效提高诊断灵敏性及特异性。
Objective: To investigate the expression of AOPP and ET1 in plasma, cord blood and placenta of pregnant women with preeclampsia and its clinical significance. Methods: From January 2012 to January 2014, 80 pregnant women with preeclampsia who were admitted to Department of Obstetrics and Gynecology, the First Affiliated Hospital of Liaoning Medical College were enrolled. Among them, 38 were mild preeclampsia, 42 were severe preeclampsia, Forty normal pregnant women treated at the same period were selected as the control group. The levels of AOPP in plasma and umbilical cord blood of the two groups were determined by spectrophotometry. The levels of ET1 in plasma and cord blood of the two groups were determined by ELISA. Results: The levels of AOPP and ET1 in plasma and umbilical cord blood of patients with mild and severe preeclampsia were significantly higher than those of control group (P <0.05), and the levels of AOPP and ET1 in plasma and cord blood of patients with severe preeclampsia were higher than those in mild Preeclampsia group (P <0.05). The levels of AOPPmRNA and ET1mRNA in placenta of patients with mild and severe preeclampsia were significantly higher than those in control group (P <0.05), and the levels of AOPPmRNA and ET1mRNA in placenta of patients with severe preeclampsia were significantly higher than those in mild preeclampsia <0.05). AOPP, ET1 combined detection of preeclampsia accuracy, sensitivity, specificity higher than AOPP or ET1 single test results (P <0.05). Conclusion: The incidence of preeclampsia is related to the high expression of AOPP and ET1 in plasma, cord blood and placenta of pregnant women. Through the combined detection of AOPP and ET1 preeclampsia can effectively improve the diagnostic sensitivity and specificity.
