[目的]探究与分析限制性液体复苏联合甘草人参汤治疗上消化道出血合并失血性休克的临床疗效。[方法]选取我院自2013年4月~2015年4月收治的60例上消化道出血合并失血性休克患者,采取随机数字表法分为常规液体复苏组与限制性液体复苏组,每组各30例,对比2组患者临床疗效、凝血酶原时间、输液量、血压、血小板计数、血红蛋白、血乳酸及BE值。[结果]限制性液体复苏组的总有效率为90.00%,常规液体复苏组的总有效率为73.33%,组间相比差异显著(χ2=5.67,P<0.05)。限制性液体复苏组的凝血酶原时间、输液量、平均血压与常规液体复苏组相比均具有显著差异(t=3.23,P<0.05;t=9.12,P<0.05;t=4.51,P<0.05)。限制性液体复苏组血小板计数、血红蛋白、血乳酸、BE值与常规液体复苏组相比均具有显著差异(t=7.84,P<0.05;t=8.11,P<0.05;t=7.94,P<0.05;t=5.22,P<0.05)。[结论]采用限制性液体复苏联合甘草人参汤治疗上消化道出血合并失血性休克的临床疗效显著,可有效缩短凝血酶原时间,输液量较少,各实验室指标改善情况显著,值得广泛推广与应用。
[Objective] To explore and analyze the clinical efficacy of restrictive fluid resuscitation combined with licorice ginseng soup in the treatment of upper gastrointestinal bleeding combined with hemorrhagic shock. [Methods] Sixty patients with upper gastrointestinal hemorrhage complicated with hemorrhagic shock who were treated in our hospital from April 2013 to April 2015 were randomly divided into two groups: conventional liquid resuscitation group and restrictive fluid resuscitation group, each group The clinical efficacy, prothrombin time, infusion volume, blood pressure, platelet count, hemoglobin, blood lactate and BE were compared between the 30 patients. [Results] The total effective rate of the liquid resuscitation group was 90.00%. The total effective rate of the conventional liquid resuscitation group was 73.33%. There was significant difference between the two groups (χ2 = 5.67, P <0.05). Prothrombin time, infusion volume and mean blood pressure in the resuscitation group were significantly different from those in the conventional liquid resuscitation group (t = 3.23, P <0.05; t = 9.12, P < 0.05). There were significant differences in platelet count, hemoglobin, blood lactate and BE values between the resuscitation group and the conventional liquid resuscitation group (t = 7.84, P <0.05; t = 8.11, P <0.05; ; t = 5.22, P <0.05). [Conclusion] The curative effect of restrictive liquid resuscitation combined with licorice ginseng soup on upper gastrointestinal hemorrhage with hemorrhagic shock is significant, which can effectively shorten the prothrombin time, reduce the volume of infusion and improve the indexes of various laboratories.
