目的:观察冠心病(CHD)患者极低密度脂蛋白(VLDL)的水平,探讨VLDL与其它脂蛋白及冠心病的关系。方法:采用常规方法测定144例经冠脉造影证实为冠心病的患者及95例无冠脉病变的阴性对照者的血浆总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白-胆固醇(HDL)、低密度脂蛋白-胆固醇(LDL)、ApoAl(载脂蛋白A1)、AtxoB(载脂蛋白B)和VLDL水平,观察两组间血脂水平的变化及VLDL与冠心病的关系。结果:(1)冠脉阳性组血浆TC、TG、LDL、ApoB水平均高于冠脉阴性组(p<0.05),HDL、ApoAl、VLDL水平在两组中无统计学意义(p>0.05)。(2)多元Logistic回归分析,结果显示对冠心病有影响的因素有LDL和TG,p<0.05。(3)多元线性回归分析,结果显示TG、LDL、ApoB和TC又与VLDL相关,p<0.05。(4)偏相关分析,结果显示TG与VLDL关系最密切(r=0.58,p<0.001),其次为LDL(r=-0.36,p<0.001)。结论:VLDL虽不是冠心病的独立危险因素,但它与冠心病的独立危险因素TG和LDL均关系密切,它通过TG和LDL间接地对冠心病的发病产生影响。
Objective: To observe the level of very low density lipoprotein (VLDL) in patients with coronary heart disease (CHD) and to explore the relationship between VLDL and other lipoproteins and coronary heart disease. Methods: Plasma concentrations of total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C) and total cholesterol in plasma of 144 patients with CHD confirmed by coronary angiography and 95 patients without coronary lesions were measured by routine methods. HDL, LDL, ApoAl (apolipoprotein A1), AtxoB (apolipoprotein B) and VLDL were measured. The changes of blood lipid levels and the relationship between VLDL and coronary heart disease were observed. The levels of TC, TG, LDL and ApoB in coronary-positive group were higher than those in coronary-negative group (p <0.05). There was no significant difference in HDL, ApoAl and VLDL between the two groups (p> 0.05) . (2) Multivariate Logistic regression analysis showed that the factors affecting coronary heart disease were LDL and TG, p <0.05. (3) Multiple linear regression analysis showed that TG, LDL, ApoB and TC were associated with VLDL, p <0.05. (4) Partial correlation analysis showed that TG was the most closely related to VLDL (r = 0.58, p <0.001), followed by LDL (r = -0.36, p <0.001). Conclusion: Although VLDL is not an independent risk factor for coronary heart disease, it is closely related to TG and LDL, which are independent risk factors of coronary heart disease. It indirectly affects the incidence of coronary heart disease through TG and LDL.
