目的检测Ⅱ型糖尿病足患者股动脉分叉处动脉内中膜厚度(IMT)和踝臂指数(ABI),并探讨其相关的临床意义。方法收集某院2009年10月~2012年10月内分泌科诊治的Ⅱ型糖尿病患者50例,其中糖尿病足患者27例,非糖尿病足患者23例。另取该院保健门诊健康体检者25例作为正常对照。多普勒超声检测IMT,血管检查仪检测ABI。结果 IMT在对照组、非糖尿病足组和糖尿病足组分别为:(0.78±0.26)、(1.08±0.32)和(1.24±0.46)mm,3组之间差异有统计学意义(P﹤0.05),其中IMT在糖尿病足组显著高于非糖尿病足组(P﹤0.05),非糖尿病足组又显著高于对照组(P﹤0.05)。ABI在3组分别为:(1.04±0.11)、(0.85±0.09)和(0.77±0.06),3组之间差异有统计学意义(P﹤0.05),ABI在糖尿病足组显著低于非糖尿病足组(P﹤0.05),非糖尿病足组又显著低于对照组(P﹤0.05)。IMT值与ABI呈显著负相关(r=-0.67,P﹤0.05)。结论糖尿病足患者具有高的IMT和低的ABI,二者的联合检测对于糖尿病足的早期诊断和防治具有重要的临床意义。
Objective To detect the IMT and ABI of femoral artery bifurcation in patients with type Ⅱ diabetic foot and to explore its clinical significance. Methods A total of 50 type Ⅱ diabetic patients diagnosed and treated in Department of Endocrinology from October 2009 to October 2012 in our hospital were collected, including 27 cases with diabetic foot and 23 cases with non-diabetic foot. Another take the hospital health clinic 25 cases of healthy people as a normal control. Doppler ultrasound detection of IMT, vascular examination ABI. Results IMT in the control group, non-diabetic foot group and diabetic foot group were (0.78 ± 0.26), (1.08 ± 0.32) and (1.24 ± 0.46) mm, respectively. The difference between the three groups was statistically significant (P0.05) , In which IMT was significantly higher in diabetic foot group than in non-diabetic foot group (P <0.05), and non-diabetic foot group was significantly higher than that in control group (P <0.05). ABI in the three groups were (1.04 ± 0.11), (0.85 ± 0.09) and (0.77 ± 0.06), respectively. The difference between the three groups was statistically significant (P <0.05) Foot group (P <0.05), non-diabetic foot group was significantly lower than the control group (P <0.05). There was a significant negative correlation between IMT and ABI (r = -0.67, P <0.05). Conclusion The patients with diabetic foot have high IMT and low ABI. The combined detection of them has important clinical significance for the early diagnosis and prevention of diabetic foot.
