目的:观察硫酸氢氯吡格雷片联合丹红注射液治疗老年颈内动脉系统短暂性脑缺血发作(TIA)所致眩晕的临床疗效。方法:选取78例颈内动脉系统TIA所致眩晕患者为研究对象,随机分为治疗组和对照组各39例。对照组采用硫酸氢氯吡格雷片治疗,治疗组在对照组的用药基础上加用丹红注射液治疗。比较2组治疗总有效率及治疗前后眩晕程度、大脑前动脉(ACA)、大脑中动脉(MCA)和大脑后动脉(PCA)血流速度的变化。结果:治疗组的治疗总有效率为94.9%,高于对照组的76.9%(P<0.05)。治疗前,2组眩晕程度评分比较,差异无统计学意义(P>0.05)。治疗7、14天,治疗组的眩晕程度评分均较治疗前下降(P<0.05),且均低于同期对照组(P<0.05)。治疗21天,2组眩晕程度评分均低于治疗前(P<0.05);治疗组的眩晕程度评分低于对照组(P<0.05)。治疗前,2组ACA、MCA及PCA血流速度比较,差异均无统计学意义(P>0.05)。对照组治疗14、21天后的MCA血流速度及治疗21天后的ACA、PCA血流速度均较治疗前升高(P<0.05)。治疗组治疗7、14及21天后的ACA、MCA及PCA血流速度均较治疗前升高(P<0.05),且均高于同期对照组(P<0.05)。结论:硫酸氢氯吡格雷片联合丹红注射液治疗老年颈内动脉系统TIA所致眩晕,临床疗效优于单纯硫酸氢氯吡格雷片治疗。
Objective: To observe the clinical efficacy of clopidogrel bisulfate tablets combined with Danhong injection in the treatment of dizziness caused by transient ischemic attack (TIA) in the aged carotid artery system. Methods: A total of 78 patients with vertigo induced by TIA in the internal carotid artery system were selected as study subjects and randomly divided into treatment group (39 cases) and control group (39 cases). The control group was treated with clopidogrel bisulfate tablets. The treatment group was treated with Danhong injection on the basis of the control group. The total effective rate and the degree of vertigo, ACA, MCA and PCA in two groups were compared before and after treatment. Results: The total effective rate of the treatment group was 94.9%, which was higher than that of the control group (76.9%, P <0.05). Before treatment, two groups of vertigo degree score comparison, the difference was not statistically significant (P> 0.05). At 7 and 14 days of treatment, the scores of vertigo in treatment group were lower than those before treatment (P <0.05), and both were lower than those in control group (P <0.05). After 21 days of treatment, the scores of vertigo in both groups were lower than those before treatment (P <0.05). The scores of vertigo in the treatment group were lower than those in the control group (P <0.05). Before treatment, blood flow velocity of ACA, MCA and PCA in two groups had no significant difference (P> 0.05). MCA blood flow velocity of control group after 14 and 21 days and ACA and PCA blood flow velocity increased 21 days after treatment (P <0.05). The blood flow velocity of ACA, MCA and PCA in treatment group after treatment for 7, 14 and 21 days were higher than those before treatment (P <0.05), and were higher than those in control group (P <0.05). Conclusion: Clopidogrel bisulfate tablets combined with Danhong injection in the treatment of dizziness caused by TIA in elderly patients with internal carotid artery have better clinical curative effect than clopidogrel bisulfate alone.
