目的通过检测2型糖尿病患者(T2DM)血清中胱氨酸蛋白酶抑制剂C(CysC)、同型半胱氨酸(HCY)、肌酐(Scr)和尿微量白蛋白(U-MA)等的水平,分析其与糖尿病肾病(DN)的关系。方法收集80例健康者和100例糖尿病患者,测定研究人群的血清中CysC、HCY、Scr和U-MA的含量,计算内生肌酐清除率(Ccr),同时检测空腹血糖(FPG)及血尿素(BUN),采用SPSS11.1系统进行了统计分析。结果 DN组CysC、HCY、Scr、U-MA、及Ccr等指标均显著高于健康组(P<0.05);FBG水平亦显著高于健康组(P<0.05)。DN组CysC与Ccr呈显著性负相关(r=-0.922,P<0.01);HCY与Ccr亦呈负相关(r=-0.903,P<0.01);Scr与Ccr亦呈负相关(r=-0.831,P<0.01)。结论 CysC、HCY联合检测可作为2型糖尿病肾病的早期诊断指标,特别是对糖尿病肾病的预防、延缓或减慢疾病的进程具有重要的临床意义。
Objective To detect serum levels of cystatin C (CysC), homocysteine (HCY), creatinine (Scr) and urine microalbumin (U-MA) in type 2 diabetic patients (T2DM) Analysis of its relationship with diabetic nephropathy (DN). Methods The serum levels of CysC, HCY, Scr and U-MA were measured in 80 healthy subjects and 100 diabetic patients. The creatinine clearance rate (Ccr) was calculated and the levels of fasting blood glucose (FPG) and blood urea nitrogen (BUN), using SPSS11.1 system for statistical analysis. Results The indexes of CysC, HCY, Scr, U-MA and Ccr in DN group were significantly higher than those in healthy group (P <0.05). FBG level was also significantly higher than that in healthy group (P <0.05). There was a significant negative correlation between Cys and Ccr in DN group (r = -0.922, P <0.01), negative correlation between HCY and Ccr (r = -0.903, P <0.01) 0.831, P <0.01). Conclusion Combined detection of CysC and HCY can be used as an early diagnostic indicator of type 2 diabetic nephropathy, especially for the prevention, delay or slowing of disease progression of diabetic nephropathy.
