过敏性紫癜患儿血清IL-9、IL-17、MMP-13的变化及意义

目的:探讨过敏性紫癜患儿血清白细胞介素-9(interleukin,IL)-9、IL-17、基质金属蛋白酶-13(metalloproteinase-13,MMP-13)的变化以及临床意义。方法:选取2016年9月至2018年9月于滨州医学院附属医院儿科收治的122例过敏性紫癜患儿作为实验组,随机选取50例健康儿童作为对照组。实验组根据有无肾脏损害分为紫癜性肾炎组(38例)和非紫癜肾炎组(84例)。抽取三组研究对象清晨空腹静脉血,采用酶联免疫吸附试验(enzyme linked immunosorbent assay,ELISA)检测IL-9、IL-17和MMP的含量,分析三组儿童血清IL-9、IL-17、MMP-13的变化。结果:实验组血清IL-9、IL-17及MMP-13均较对照组明显升高[(30.58±7.04) pg/mL比(26.74±6.28)pg/mL,(90.61±22.17)pg/mL比(80.06±20.83)pg/mL,(216.27±35.58)pg/mL比(156.83±34.36)pg/mL,n t值分别为3.641,5.580和6.023,n P值均<0.05],差异具有统计学意义。紫癜性肾炎组IL-9、IL-17及MMP-13水平均较非紫癜性肾炎组和对照组明显升高[(32.92±5.37) pg/mL比(29.30±4.46)pg/mL比(26.74±6.28)pg/mL,(96.75±17.06)pg/mL比(85.14±18.83 pg/mL)比(80.06±20.83)pg/mL,(227.81±33.02)pg/mL比(188.85±32.14)pg/mL比(156.83±34.36)pg/mL,n F值分别为10.283、12.627、20.074,n P值均<0.05],差异具有统计学意义。n 结论:IL-9和IL-17增高导致抗原抗体复合物增多,机体出现血管炎症,MMP-13的增高可破坏血管结构,造成血管损伤,提示这三者可能均参与了过敏性紫癜的发病过程。“,”Objective:To investigate the changes and clinical significance of serum interleukin(IL)-9, IL-17 and metalloproteinase-13(MMP-13) in children with allergic purpura.Methods:Total of 122 children with allergic purpura were collected from Binzhou Medical College Affiliated Hospital from September 2016 to September 2018 as the experimental group and 50 healthy children were randomly selected as the control group.The experimental group was divided into purpura nephritis group(38 cases) and non-purpura nephritis group(84 cases) according to the absence of renal damage.Fasting venous blood samples of three groups were collected in the morning.The contents of IL-9, IL-17 and MMP-13 were determined by enzyme linked immunosorbent assay(ELISA). The changes of serum IL-9, IL-17 and MMP-13 in the three groups were also analyzed.Results:Serum IL-9, IL-17 and MMP-13 in the experimental group were significantly higher than those in the control group[(30.58±7.04) pg/mL vs(26.74±6.28)pg/mL, (90.61±22.17)pg/mL vs(80.06±20.83)pg/mL, (216.27±35.58)pg/mL vs(156.83±34.36)pg/mL, n t values were 3.641, 5.580 and 6.023 respectively, all n P values <0.05]. The levels of IL-9, IL-17 and MMP-13 in the purpura nephritis group were higher than those in the non-purpura nephritis group and the control group[(32.92±5.37) pg/mL vs(29.30±4.46)pg/mL vs(26.74±6.28)pg/mL, (96.75±17.06)pg/mL vs(85.14±18.83)pg/mL vs(80.06±20.83 pg/mL), (227.81±33.02)pg/mL vs(188.85±32.14)pg/mL vs(156.83±34.36)pg/mL, n F values were 10.283, 12.627 and 20.074 respectively, all n P values<0.05].n Conclusion:The increase of IL-9, IL-17 leads to the increase of antigen-antibody complex and vascular inflammation, and the increase of MMP-13 can damage the vascular structure and cause vascular injury, which suggests that IL-9, IL-17 and MMP-13 may be involved in the pathogenesis of allergic purpura.
