目的本文主要针对高效价冷凝集素所致血型鉴定困难进行深入的分析,为临床提供参考。方法对200例含高效价冷凝集素患者的临床资料进行分析,同时进行抗人球蛋白试验、冷凝集素效价测定以及红细胞ABO血型鉴定等血型血清学检测,探讨血型鉴定困难的原因。结果当冷凝集素的效价在<1∶2~1∶32,不会出现假凝集现象。当冷凝集素的效价>1∶64,冷凝集素就会感染血型的鉴别测定。若对高效价的冷凝集素效的血液进行血型的确定,就容易导致血型判断出错。如果对高效价冷凝集素血液采取反向定型时,易将A、B、AB血型错误鉴定为O型。结论由于患者体内冷凝集素的效价在增高时,会对ABO血型鉴定的结果造成一定的影响,所以临床对于疑难血型的鉴定标本,应尽量选择科学的鉴别试验方法进行排除,最终正确鉴定ABO血型。
Objective In this paper, mainly for high-potency cold agglutinin-induced blood type identification difficulties in-depth analysis for the clinical reference. Methods The clinical data of 200 patients with high titer of cold agglutinin were analyzed. At the same time, anti-human globulin test, cold agglutinin titer and red blood cell ABO blood group were used to determine the blood type. Results When titer of cold agglutinin was in the range of <1: 2 to 1:32, no false agglutination occurred. When the titer of cold agglutinin> 1:64, cold agglutinin will be infected with blood type identification determination. If the high titer of blood coagulation efficiency of blood type determination, it is easy to cause blood type to determine the error. If the anti-agglutinin high titer blood to take reverse stereotypes, it is easy to A, B, AB mistakenly identified as O blood type. Conclusion Because the cold agglutinin titer in patients with increased, it will have some impact on the ABO blood group identification results, so clinical specimens for the identification of difficult blood type, should try to choose a scientific differential test method to exclude the final correct identification of ABO blood type.
