目的 建立中晚孕期正常胎儿胼胝体-小脑幕夹角(CTA)正常参考值范围,并评估其在单纯性枕大池增宽(MCM)及DWM中的应用价值.方法 采用三维超声容积对比成像(3DUS-VCI)技术测量350例21~32孕周正常单胎胎儿CTA,分析其与孕周的关系,并按照上述方法对14例MCM和12例DWM胎儿进行评估.结果(1)正常胎儿CTA与孕周不存在明显相关(r=0.014,P>0.05);(2)3DUS-VCI测量CTA有良好的重复性及一致性;(3)MCM胎儿CTA与正常胎儿组无明显差异,DWM胎儿CTA较正常组及MCM组明显减小(32.76±6.23)°vs(55.82±5.32)°,t=25.12,P<0.05;(32.76±6.23)°vs(56.66±4.18)°,t=26.04,P<0.05.结论 运用3DUS-VCI技术测量胎儿CTA的方法是可行的,有助于单纯性枕大池增宽和Dandy-Walker畸形的鉴别诊断.“,”Objective To establish the ranges of normal reference values of fetal corpus callosum-tentorium angle (CTA) in normal fetuses, and to evaluate their clinical values in the diagnosis of mega cisterna magna (MCM) and Dandy-Walker malformation (DWM).Methods Totally 350 normal singleton fetuses between 21 and 32 weeks of gestation were enrolled.Three dimensional ultrasonography volume contrast imaging (3 DUS-VCI) was used to measure their CTA, the correlations of CTA with gestational age were investigated.The CTA of 14 fetuses with MCM and 12 fetuses with DWM were also measured and were compared with those of the normal fetuses.Results In these normal fetuses, the CTA showed no correlations with gestational age (r=0.014, P>0.05).The CTA of 14 fetuses with MCM were not significantly different from those of normal fetuses, while the CTA of 12 fetuses with DWM were significantly smaller than those of normal fetuses and the fetuses with MCM (32.76±6.23) °vs (55.82±5.32) °, t=25.12, P<0.05; (32.76±6.23) °vs (56.66±4.18) °, t=26.04, P<0.05.The repeatability of the same physician and the consistency between two different physicians were good.Conclusions VCI is feasible for measuring the CTA of fetuses.The measuring of the CTA can help differentiate MCM from DWM.
