目的:探讨转化生长因子-β1(TGF-β1)和纤维粘连蛋白(FN)在压力性尿失禁(SUI)大鼠模型阴道前壁中的表达及意义。方法将30只健康雌性SD大鼠随机分为3组,A组作为对照组不做建模处理;B组仅行阴道扩张;C组行阴道扩张加卵巢切除。术后4周测定腹压漏尿点压(ALPP)及最大膀胱容量(MBC),评估模型建立是否成功。4周后处死大鼠,取阴道前壁组织用免疫组化法和实时荧光定量PCR检测TGF-β1和FN的表达。结果尿流动力学(ALPP、MBC):B、C组较A组明显降低,且各组之间比较差异均有统计学意义(P<0.001);C组TGF-β1 mRNA与蛋白质的表达明显低于A、B组(P0.05);C组FN mRNA及蛋白质的表达明显高于A、B组(P<0.001),且A、B两组之间比较差异有统计学意义(P<0.001)。结论 SUI大鼠阴道前壁中TGF-β1的低表达和FN的高表达可能与盆底支持结构的退行性病变密切相关。“,”ObjectiveTo investigate the expression of the transforming growth factor-β1(TGF-β1)and fibronectin(FN)in anterior vaginal wall of stress urinary incontinence model rats.Methods Thirty healthy female Sprague Dawley rats were randomly divided into three groups: group A as a control group without modeling process; group B were received vaginal dilation and group C were underwent simulated birth trauma induced by vaginal distension and ovariectomy.After 4 weeks,we assessed the model success or not by measuring abdominal leak point pressure measurement(ALPP)and the maximum bladder capacity(MBC).All rats were sacrificed after 4 weeks.The expression of TGF-β1and FN in anterior vaginal wall tissue was examined by real-time reverse transcriptasepolymerase chain reaction and immunohistochemis-try.ResultsCompared to controls, ALPP and MBC were significantly decreased in group B and group C.There was a significant difference of the ALPP and MBC between two groups (P<0.001);The expression of TGF-β1mRNA and protein in group C was significantly reduced than group A and group B(P0.05);The expression of FN mRNA and protein in group C was significantly increased than group A and group B(P<0.001),and the difference between two groups were significant(P<0.001).Conclusion The decreased expression of TGF-β1 and the highly expression of FN in anterior vaginal wall of stress urinary incontinence model rats may be closely associated with degenerative diseases of pelvic floor support structure.
