目的评价增强减影 MRI 在椎体转移瘤诊断中的应用价值。方法对30例44个椎体转移瘤进行常规 MR 序列扫描,同时使用对比增强剂钆喷替酸葡甲胺进行增强显影,并对增强前后的 T_1WI 进行减影处理。通过对椎体肿瘤影像的清晰度、信号强度比(SIR)和噪声比(NR)的比较,评价增强减影 MRI 在椎体转移瘤上的诊断价值。结果增强减影 MRI 及增强 T_1、SE T_1、T_2、反转恢复序列(STIR)图像的 SIR 值分别为2.93、1.15、1.16、1.26、1.69。增强减影 MRI 及增强 T_1、SE T_1、SET_2、STIR 图像的 NR 值分别为0.98、5.25、3.44、4.56、23.32。增强减影 MR 图像的 SIR 和 NR 较常规MRI 图像差异均有统计学意义(P<0.05),其信号对比度及图像质量均较好。结论与常规 MR 序列图像相比,增强减影 MRI 对椎体转移瘤病灶的细节显示得更清楚,病灶与背景的对比度更明显,图像质量更清晰,在椎体转移瘤的临床诊断上有重要的应用价值,可以在增强显影后常规应用。
Objective To evaluate the value of enhanced subtraction MRI in the diagnosis of vertebral metastases. Methods Totally 44 vertebral metastases of 30 cases were scanned by conventional MR sequence, and contrast enhancement agent gadolinium was used to enhance the development of TlWI before and after enhancement. The diagnostic value of enhanced subtraction MRI in vertebral metastases was evaluated by comparing the sharpness, signal intensity ratio (SIR) and noise ratio (NR) of vertebral body tumor images. Results The SIR values of enhanced MRI, enhanced T 1, SE T 1, T 2 and inverted recovery sequence (STIR) images were 2.93, 1.15, 1.16, 1.26 and 1.69, respectively. The enhanced NR and enhanced T_1, SE T_1, SET_2 and STIR images were 0.98, 5.25, 3.44, 4.56 and 23.32, respectively. Compared with conventional MRI images, the SIR and NR of enhanced MR images were statistically significant (P <0.05), and the signal contrast and image quality were better. Conclusion Compared with conventional MR sequence images, enhanced subtraction MRI showed more clearly the details of the lesion of vertebral metastases, the contrast between the lesion and the background was more obvious and the image quality was more clear, which was significant in the clinical diagnosis of vertebral metastases The application value, can enhance the conventional application after development.
