目的:探讨螺旋CT血管造影及三维图像重建(3D-SCTA)在颅内和头颈部病变诊断中的价值。材料和方法:98例头颈部病变患者接受了3D-SCTA检查,其中包括颅内动脉瘤32例,颅内AVM 5例,颈动脉粥样硬化35例,脑膜瘤11例,硬膜型海绵状血管瘤2例,颅底神经纤维瘤3例,颈部肿块6例,颈椎椎管内神经纤维瘤4例。结果:全部患者顺利经受螺旋CT血管造影检查,35例颈动脉粥样硬化均显示清楚,且能够检出附壁钙化性动脉粥样硬化斑块,对颈动脉粥样硬化程度的诊断有较高的准确性。32例颅内动脉瘤中30例获清晰显示,动脉瘤的瘤体大小、瘤颈情况、瘤轴的指向、载瘤动脉以及动脉瘤与颅区骨性组织的空间关系和动脉瘤与相邻血管分支的关系均能获得较满意的显示。16例颅内肿瘤(包括11例脑膜瘤,3例颅底神经纤维瘤,2例硬膜型海绵状血管瘤)与Willis环及其分支的关系,与硬膜窦静脉的关系以及与颅底诸孔的关系皆直观显示。6例颈部肿瘤的大小、形态位置及其与颈动脉、颈静脉血管的空间关系都清晰地显示。4例颈椎管内神经纤维瘤生长范围、形态和与骨性颈椎的关系得到直观地显示。三维图像的伪彩色显示、模拟手术视角设计,为病变的定位诊断和治疗决策提供了有效的帮助
Objective: To investigate the value of spiral CT angiography and three-dimensional image reconstruction (3D-SCTA) in the diagnosis of intracranial and head and neck lesions.
