目的:比较无晶状体后囊支撑的人工晶状体两点式睫状沟悬吊术与四点式睫状沟悬吊术的临床效果。方法:回顾性分析2018年5月至2019年8月郑州大学第一附属医院收治各种原因导致的无晶状体后囊支撑行人工晶状体睫状沟悬吊术患者59例(60只眼)的临床资料。其中两点式人工晶状体睫状沟悬吊术组29例(30只眼)，四点式人工晶状体睫状沟悬吊术组30例(30只眼)，所有患眼均随访3个月以上，观察比较两组视力、散光度及人工晶状体位置。结果:术后两组3个月视力均提高，较术前视力相比差异均有统计学意义(两点式组：n t＝8.355，n P＝0.000；四点式组：n t＝7.227，n P＝0.000)。术后3个月视力两组间差异无统计学意义(n t＝－0.955，n P＝0.348)。四点式组较两点式组术后散光度变化小(n t＝2.431，n P＝0.018)，术后人工晶状体偏斜较少(n P＝0.026)。n 结论:人工晶状体两点式悬吊与四点式悬吊均可提高患者术后最佳矫正视力。四点式悬吊术后散光等症状更轻，且术后更不易发生人工晶状体偏斜。“,”Objective:To compare the clinical efficacy between two-point and four-point ciliary sulcus suspension of intraocular lens in aphakic eye without posterior lens capsule.Methods:Clinical data of 60 eyes of 59 patients without posterior capsular support caused by various reasons from May 2018 to Aug. 2019 in the First Affiliated Hospital of Zhengzhou University were analyzed retrospectively.All cases received ciliary sulcus suspension of intraocular lens.Thirty eyes of 29 cases were treated with two-point of ciliary sulcus suspension as the two-point group and 30 eyes of 30 cases were treated with four-point of ciliary sulcus as the four-point group.All patients were followed up for more than 3 months.The visual acuity, astigmatism degree and position of intraocular lens were observed and compared between the two groups.Results:The postoperative visual acuity of two groups were improved at 3 months after surgery. The differences of visual acuity were statistically significant in the two groups between before and after surgery(the two-point group: n t=8.355, n P=0.000; the four-point group: n t=7.227, n P=0.000). There was no statistically significant difference between the two groups(n t=-0.955, n P=0.348). The astigmatism degree change after surgery in the four-point group was lower than that in the two-point group, and the difference was statistically significant(n t=2.431, n P=0.018). Intraocular lens tilt in the four-point group was less than that in the two-point group(n P=0.026).n Conclusion:Two-point and four-point ciliary sulcus suspension of the intraocular lens can both improve the best corrected visual acuity.The symptoms of astigmatism after four-point suspension surgery will be milder, and postoperative intraocular lens tilt will be less likely to occur.
