目的:探讨血红蛋白自动分析仪在糖尿病合并高血脂患者糖化血红蛋白(Hb A1C)及血液流变学联合检测中的应用。方法:选取84例2型糖尿病伴高脂血症患者纳入观察组,同时选择84名健康体检者为对照组,采用血红蛋白自动分析仪对两组进行血液流变学及血糖检测,并进行对比,分析联合检测Hb A1C及血液流变学对糖尿病合并高血脂患者的临床意义。结果:观察组与对照组血脂指标检测结果比较,观察组患者血清胆固醇(TC)、甘油三酷(TG)及低密度脂蛋白(LDL)等水平均明显高于对照组,差异显著(t=9.70,t=11.37,t=29.60;P<0.05),而高密度脂蛋白(HDL)无统计学差异;观察组与对照组HbA1C及血液流变学5项指标检测结果比较,观察组HbA1C、全血高切黏度及全血低切黏度等水平均明显高于对照组,差异显著(t=3.16,t=3.32,t=3.08;P<0.05);观察组全血低切黏度、高切黏度与HbA1C间呈高度正相关(r=0.948,r=0.951;P<0.01);血浆黏度与HbA1C间也存在明显相关性(r=0.863,P<0.01)。结论:联合检测血液流变学指标与HbA1c可有效反映糖尿病合并高血脂患者病情变化,对临床诊治意义重大,值得进一步推广使用。
Objective: To investigate the application of automatic hemoglobin analyzer in the combined detection of glycosylated hemoglobin (Hb A1C) and hemorrheology in diabetic patients with hyperlipidemia. Methods: Eighty-four patients with type 2 diabetes mellitus and hyperlipidemia were enrolled in the observation group and 84 healthy subjects were selected as the control group. Hemorrheology and blood glucose were detected by automatic hemoglobin analyzer. To analyze the clinical significance of combined detection of Hb A1C and hemorheology in patients with diabetes complicated with hyperlipidemia. Results: Compared with the control group, the levels of serum cholesterol, triglyceride and low density lipoprotein (LDL) in the observation group were significantly higher than those in the control group (t = 9.70, t = 11.37, t = 29.60; P <0.05), while there was no significant difference in HDL between the observation group and the control group. The HbA1C and hemorheology indexes in observation group and control group were compared. The levels of whole blood high shear viscosity and whole blood low shear viscosity were significantly higher than those of the control group (t = 3.16, t = 3.32, t = 3.08, P <0.05) There was a positive correlation between viscosity and HbA1C (r = 0.948, r = 0.951; P <0.01). There was also a significant correlation between plasma viscosity and HbA1C (r = 0.863, P <0.01). Conclusion: The combined detection of hemorheology index and HbA1c can effectively reflect the changes of patients with diabetes mellitus and hyperlipidemia, which is of great significance to clinical diagnosis and treatment and is worth further promotion and use.
