目的 观察和评价含力克肺疾方案治疗老年复治菌阳肺结核的疗效及安全性。方法 将 115例老年复治菌阳肺结核患者随机分入治疗组 (A组 )和对照组 (B1组、B2组 )。化疗方案 :A组以力克肺疾为主组成 3DZL2 E/ 5DL2 E ;B1组 3HPZL2 / 5HL2 E(简称HP方案 ) ;B2组 3HZL2 ES/ 5HL2 E(简称SH方案 )。采用全程监督化疗管理 ,按各自方案实施化疗。结果 疗程结束时 ,A、B1、B2组痰菌阴转率分别 92 %、72 %、71% ,A组显著高于B1组和B2组 (P <0 0 5 ) ;病灶显效率分别为 75 %、5 0 %、5 0 % ,空洞闭合率分别为 85 %、46 %、5 0 % ,A组显著高于B1组和B2组 (P <0 0 5 ) ;不良反应发生率分别为17%、5 8%、18% ,A组和B2组显著低于B1组 (P <0 0 1) ;2年细菌学复发率分别为 5 %、19%、18% ,A组显著低于B1组和B2组 (P <0 0 1)。结论 含力克肺疾方案治疗老年复治菌阳肺结核的近远期疗效均优于HP方案和SH方案 ,且不良反应发生率低 ,使用安全 ,具有临床推广应用价值
Objective To observe and evaluate the efficacy and safety of Lectacril pulmonary disease regimen in the treatment of geriatric bacillary positive pulmonary tuberculosis. Methods A total of 115 elderly patients with convalescent pulmonary tuberculosis were randomly divided into treatment group (group A) and control group (group B1 and group B2). Chemotherapy regimen: Group A consisted of 3DZL2 E / 5DL2 E with Lectra Pulmonary Disease; Group B1 3HPZL2 / 5HL2 E (referred to as HP regimen); Group B2 3HZL2 ES / 5HL2 E (abbreviated SH regimen). With full supervision of chemotherapy management, according to the implementation of chemotherapy programs. Results At the end of treatment, sputum negative conversion rates were 92%, 72% and 71% in group A, B1 and B2, respectively, significantly higher in group A than those in group B1 and group B2 (P <0 05) %, 50%, 50% and void closure rates were 85%, 46% and 50% respectively in group A, which were significantly higher in group A than those in group B1 and group B2 (P <0.05). The rates of adverse reactions were 17 %, 58%, 18% in group A and B2 were significantly lower than those in group B1 (P <0.01). The two-year bacteriological recurrence rates were 5%, 19% and 18% Group and B2 group (P <0.01). CONCLUSION: The short-term and long-term effects of Lectacril disease regimen in the treatment of geriatric pulmonary tuberculosis in elderly patients are better than that of HP regimen and SH regimen, with low incidence of adverse reactions, safe use and clinical application value
