目的 观察 2型糖尿病患者钙、磷代谢及相关激素的变化及对骨代谢的影响。方法 测定 2 0例健康对照者和 6 0例 2型糖尿病患者血甲状旁腺素 (PTH)、降钙素 (CT)、2 5羟维生素 D3[[2 5 (OH) D3]]、环磷酸腺苷 (c AMP)和血、尿钙 (Ca)、磷 (P)、镁 (Mg) ,糖基化血红蛋白 (Hb Alc)、2 4h尿糖和 2 4h尿白蛋白定量等指标。结果 2型糖尿病患者血PTH水平显著高于对照组 (P<0 .0 1 ) ,CT水平显著低于对照组 (P<0 .0 1 ) ,尿 Ca、尿 Mg排泄量显著高于对照组 (P<0 .0 1和 P<0 .0 5 ) ;PTH水平与 Hb Alc和尿 Ca呈显著正相关 (r=0 .40 ,P<0 .0 5和 r=0 .83,P<0 .0 0 1 ) ,与 CT呈显著负相关 (r=- 0 .74,P<0 .0 1 )。结论 2型糖尿病患者存在明显 Ca,Mg代谢异常和继发性 PTH,CT改变是造成骨质疏松的主要原因 ,这种变化在糖尿病早期更明显
Objective To observe the changes of calcium and phosphorus metabolism, related hormones and the effects on bone metabolism in type 2 diabetic patients. Methods Blood samples were collected from 20 healthy controls and 60 patients with type 2 diabetes mellitus (PTH), calcitonin (CT), 25 hydroxyvitamin D3 [[25 (OH) D3] The levels of c AMP, blood calcium, phosphorus, magnesium, Hb Alc, 24 h urine glucose and 24 h urinary albumin were measured. Results The level of PTH in type 2 diabetic patients was significantly higher than that in control group (P <0.01), the level of CT was significantly lower than that in control group (P0.01), and the urinary Ca and urinary Mg excretion were significantly higher than those in control group (P <0.01 and P <0.05). The level of PTH was positively correlated with Hb Alc and urinary Ca (r = 0.40, P <0.05 and r = 0.83, P < 0. 0 0 1), and negatively correlated with CT (r = - 0.74, P <0.01). Conclusion There is obvious Ca and Mg metabolism abnormalities and secondary PTH in patients with type 2 diabetes. The change of CT is the main reason of osteoporosis, which is more obvious in the early stage of diabetes
