目的探讨糖尿病患者视网膜中央动脉(CRA)的血流动力学改变。方法应用彩色多普勒对糖尿病患者92只眼和正常对照组60眼的CRA收缩期流速(PSV)、舒张期流速(EDV)、搏动指数(PI)、阻力指数(RI)和血流加速时间(AT)进行对比分析。并将糖尿病患者按病程长短及血糖水平分组,分析上述参数。结果糖尿病组CRA的PSV、EDV低于正常组,PI、RI、AI高于正常组。病程≥10年组CRA的PSV、EDV低于病程<10年组,PI、RI高于病程<10年组。三个月平均空腹血糖≥8mmol/L组CRA的PSV、EDV低于血糖<8mmol/L组,PI、RI高于血糖<8mmol/L组。结论糖尿病患者视网膜循环阻力增加,血流速度下降,血流灌注不良,且病程愈长、平均血糖水平愈高,视网膜糖尿病血管病变愈明显。
Objective To investigate the hemodynamic changes of central retinal artery (CRA) in diabetic patients. Methods Color Doppler echocardiography was used to examine the changes of CRA systolic velocity (PSV), diastolic velocity (EDV), pulsatility index (PI), resistance index (RI) and blood flow acceleration time in 92 eyes of diabetic patients and 60 normal controls (AT) for comparative analysis. Diabetic patients were grouped according to duration of disease and blood glucose levels, and the above parameters were analyzed. Results The PSV and EDV of CRA in diabetic group were lower than those in normal group, PI, RI and AI were higher than those in normal group. The PSV and EDV of CRA were lower than those of <10 years in the course of more than 10 years, PI and RI were higher than those of <10 years. The average PSV and EDV of CRA in the group with fasting blood glucose> 8mmol / L in three months were lower than those in group with <8mmol / L blood glucose, PI, RI were higher than those in group with blood glucose <8mmol / L. Conclusion Diabetic patients with increased retinal circulation resistance, decreased blood flow velocity, poor perfusion, and the longer the duration, the higher the average blood glucose level, the more obvious diabetic retinopathy.
