目的研究钠钾镁钙葡萄糖注射液在老年患者经尿道前列腺电切术(TURP)补液中的应用效果。方法选取青岛市海慈医疗集团2013年2—10月行TURP患者60例,按照随机数字表法分为葡萄糖注射液组(试验组)和乳酸钠林格注射液组(对照组),各30例。测量并比较两组患者术前(T1)、术后30min(T_2)、术毕(T_3)的平均动脉压(MAP)、血氧饱和度(SpO_2)、心率(HR)、中心静脉压(CVP)、血糖水平及电解质(Na~+、K~+、Cl~-、Mg~(2+)、Ca~(2+))水平。结果两组患者T_1、T2、T3时相MAP、SpO_2、HR、CVP比较,差异无统计学意义(P>0.05);两组患者T_2、T_3时相MAP均低于T1时相,HR均高于T1时相,差异有统计学意义(P<0.05)。两组患者T_1、T_2时相的血糖水平比较,差异无统计学意义(P>0.05);两组患者各时相Na~+、K~+、Cl~-、Ca~(2+)水平比较,差异无统计学意义(P>0.05)。试验组T3时相血糖水平高于T_1时相,对照组T_2、T_3时相Mg~(2+)水平低于T1时相,差异有统计学意义(P<0.05);试验组T_3时相血糖、Mg~(2+)水平高于对照组,差异有统计学意义(P<0.05)。结论钠钾镁钙葡萄糖注射液的电解质配比及渗透浓度更接近于正常机体细胞外液水平,对于行TURP的老年患者,术中静脉滴注钠钾镁钙葡萄糖注射液更有利于维持电解质和代谢平衡。
Objective To study the effect of sodium potassium magnesium calcium and glucose injection in rehydration of transurethral resection of prostate (TURP) in elderly patients. Methods Sixty TURP patients were selected from Qingdao Haici Medical Group from January to October in 2013. They were divided into glucose injection group (experimental group) and Lactated Ringer’s injection group (control group) according to the random number table, and 30 . The mean arterial pressure (MAP), oxygen saturation (SpO_2), heart rate (HR) and central venous pressure (CVP) were measured and compared between the two groups before operation (T1), 30min after operation (T_2) ), Blood glucose level and electrolytes (Na ~ +, K ~ +, Cl ~ -, Mg ~ (2 +), Ca ~ (2+)). Results There was no significant difference in MAP, SpO_2, HR and CVP between the two groups at T_1, T_2 and T_3 (P> 0.05). The MAP of T_2 and T_3 in both groups were lower than those in T1 and HR At T1, the difference was statistically significant (P <0.05). There were no significant differences in blood glucose levels between the two groups at T_1 and T_2 (P> 0.05). The levels of Na ~ +, K ~ +, Cl ~ - and Ca ~ (2+) , The difference was not statistically significant (P> 0.05). The blood glucose level of T3 in test group was higher than that in T_1 phase, while the level of Mg 2+ in T_2 and T_3 phase of control group was lower than that in T1 phase (P <0.05) , Mg 2+ higher than the control group, the difference was statistically significant (P <0.05). Conclusion Electrolyte ratio and osmolality of sodium, potassium, magnesium and glucose-glucose injection are closer to normal levels of extracellular fluid. For elderly patients undergoing TURP, intraoperative intravenous infusion of sodium potassium magnesium calcium and glucose injection is more conducive to maintaining electrolyte and Metabolic balance.
