研究血浆内皮素-1(ET-1)的浓度与小儿先天性室间隔缺损伴肺动脉高压的关系。42例小儿室间隔缺损心内直视修补术病例,围术期采用放射免疫法测定血浆EF-1。结果:血浆ET-1明显高于正常,而主动脉血样ET-1略高于肺动脉血样中ET-1。肺动脉高压组术前ET-1明显高于肺动脉压力正常组(P<0.01)。逐步回归发现ET-1与肺循环流量和脉动脉压力呈线性相关。动态观察发现体外循环结束时ET-1较转流前升高,术后24h则降至低于术前水平。结论:结果表明在小儿室间隔缺损中ET-1与肺动脉高压的发生密切相关。
To investigate the relationship between plasma endothelin-1 (ET-1) concentration and congenital ventricular septal defect in children with pulmonary hypertension. 42 cases of ventricular septal defect open heart surgery cases, perioperative radioimmunoassay of plasma EF-1. Results: Plasma ET-1 was significantly higher than normal, while ET-1 in aortic blood was slightly higher than ET-1 in pulmonary arterial blood. Preoperative ET-1 in pulmonary hypertension group was significantly higher than that in normal pulmonary hypertension group (P <0.01). A stepwise regression found that ET-1 was linearly correlated with pulmonary circulation flow and pulse pressure. Dynamic observation found that ET-1 increased at the end of cardiopulmonary bypass compared with that before commutation, and then decreased to below the preoperative level at 24h after CPB. CONCLUSIONS: The results show that ET-1 is associated with pulmonary hypertension in children with ventricular septal defect.
