目的探讨延伸护理服务结合自护模式对长期卧床患者压疮的影响。方法选择在本院接受治疗出院的175例行动不便需长期卧床的患者,根据患者出院后护理方式的不同分为实验组与对照组,实验组在患者出院后实施延伸护理服务结合自护模式,在患者出院前制定相应的出院计划,定期上门随访,指导患者及家属注意事项并予以健康宣教,比较2组患者健康知识、自我护理能力及压疮发生率。结果实验组防治知识、用药知识、自我管理、运动、心理、饮食、按时复查的健康知识评分显著优于对照组(P<0.05);实验组自我概念、自我责任感、自我护理技能的自护能力评分显著优于对照组(P<0.05)。实验组压疮发生率为15.91%,显著低于对照组的压疮发生率57.47%(P<0.05);实验组患者访问门诊次数显著低于对照组(P<0.05)。实验组患者压疮愈合率显著高于对照组,压疮再次发生率显著低于对照组(P<0.05)。结论延伸护理服务结合自护模式应用于长期卧床患者,不仅能提高患者健康知识水平、自护能力,还能有效降低压疮的发生率。
Objective To explore the effect of extended nursing service combined with self-care mode on pressure ulcer in long-term bedridden patients. Methods A total of 175 hospitalized patients with long-term bed rest who were discharged during hospitalization were divided into experimental group and control group according to their different ways of nursing after discharge. The experimental group were given extended nursing service combined with self-protection mode after discharge, Discharge planning was made before the patients were discharged from hospitals. Regular follow-up visits were conducted to guide the patients and their family members in their health matters. The health knowledge, self-care ability and the incidence of pressure ulcers in both groups were compared. Results The score of health knowledge in prevention group, medication knowledge, self-management, exercise, psychology, diet and on-time review were significantly higher in experimental group than those in control group (P <0.05). Self-concept, self-responsibility, self-care ability The score was significantly better than the control group (P <0.05). The incidence of pressure ulcers in the experimental group was 15.91%, which was significantly lower than that in the control group (57.47%, P <0.05). The number of visits in the experimental group was significantly lower than that in the control group (P <0.05). The rate of pressure ulcer healing in the experimental group was significantly higher than that in the control group, and the recurrence rate of pressure ulcer was significantly lower than that in the control group (P <0.05). Conclusion Extending nursing service combined with self-care mode in long-term bedridden patients can not only improve patients’ health knowledge, self-care ability, but also effectively reduce the incidence of pressure ulcers.
