目的探讨术前和术后早期给予肠内营养对老年结直肠癌患者术后免疫功能的影响。方法将87例老年结直肠癌患者,随机分为治疗组和对照组。治疗组44例,术前1周及手术24h早期给予肠内营养支持治疗,对照组43例,术后24h早期给予肠内营养支持治疗。结果治疗组术前d1CD3+、CD4+、CD16+CD56+和CD4+/CD8+比值及IgA、IgG、IgM水平与对照组术前比较均明显升高,CD8+比例明显下降;术后d9CD3+、CD4+、CD16+CD56+和CD4+/CD8+比值及I-gA、IgG、IgM水平与对照组术后d9比较均明显升高,CD8+比例明显下降,差异均有统计学意义(P﹤0.05)。治疗组和对照组给予肠内营养治疗后未出现明显不良反应。结论对老年结直肠癌患者术前及术后早期给予肠内营养治疗能提高患者术前的免疫功能,促进患者术后的恢复。
Objective To investigate the effect of preoperative and postoperative enteral nutrition on immune function in elderly patients with colorectal cancer. Methods 87 elderly patients with colorectal cancer were randomly divided into treatment group and control group. Forty-four patients in the treatment group received enteral nutrition support one week before operation and 24 hours after operation, and 43 patients in the control group received enteral nutrition support in the early 24 hours after operation. Results The preoperative levels of d1CD3 +, CD4 +, CD16 + CD56 + and CD4 + / CD8 + and the levels of IgA, IgG and IgM in the treatment group were significantly higher than those in the control group The ratio of CD4 + / CD8 + and the levels of I-gA, IgG and IgM in the control group were significantly higher than those in the control group (P <0.05), but the proportion of CD8 + was significantly decreased. The treatment group and the control group were given no significant adverse reactions after enteral nutrition treatment. Conclusions The preoperative and early postoperative enteral nutrition therapy for elderly patients with colorectal cancer can improve the preoperative immune function and promote postoperative recovery.
