目的 :分析影响磁共振前列腺波谱扫描技术成像质量的因素,提出质量控制措施,提高前列腺波谱检查的成功率和波谱成像质量。方法:回顾性分析2009年10月至2015年12月间1 255例前列腺磁共振波谱分析(magnetic resonance spectroscopy,MRS)患者资料(所有患者均有前列腺疾病临床症状,检查资料齐全且经病理证实),MRS扫描采用多体素3D化学位移成像技术。扫描结束后使用Spectroscopy前列腺波谱专用后处理软件对MRS进行规范化后处理,观察MRS波谱质量,若有基线杂乱、信噪比低下等情况,找出原因重新扫描,直到满足诊断要求。结果:1 218例患者准备充分,定位准确,参数设置合理,后处理过程规范,所得谱线基线平稳,化合物位置准确;1例患者因扫描床位模式不正确导致检查失败;9例患者因准备不充分,未能排尿、排便导致谱线基线不稳、波谱谱线形态差;26例患者因匀场效果不好导致谱线基线不稳、谱线杂乱;1例患者因后处理不规范导致良性疾病谱线出现恶性疾病的代谢产物。结论:患者准备充分、定位规范准确、参数设置合理、匀场效果佳及规范的后处理是保证前列腺MRS扫描成功及结果可靠的关键,对于提高前列腺MRS的质量有重要意义。
OBJECTIVE: To analyze the factors influencing the imaging quality of MRS and propose quality control measures to improve the success rate and quality of spectral imaging. Methods: A retrospective analysis of 1 255 patients with magnetic resonance spectroscopy (MRS) data from October 2009 to December 2015 (all patients had clinical symptoms of prostate disease, the examination data were complete and pathologically confirmed) , MRS scanning using multi-voxel 3D chemical shift imaging technology. After the scanning, we use Spectroscopy prostate post-processing software to normalize the MRS and observe the quality of the MRS spectrum. If the baseline is cluttered and the signal-to-noise ratio is low, find out the reason to rescan until the diagnosis is satisfied. Results: A total of 1 218 patients were adequately prepared, positioned accurately, the parameters were set reasonably, the post-treatment process was standardized, the baseline of the obtained spectrum was stable, and the compound location was accurate. One patient failed the examination due to the improper scanning of the bed model. Full, failed to urinate, bowel movements caused by stool baseline instability, spectral line shape poor; 26 patients due to poor shimming results in poor baseline spectral line, spectral disorder; 1 patient due to improper post-processing leads to benign The spectrum of the disease presents a metabolite of a malignant disease. Conclusion: It is of great significance to improve the quality of MRS in the prostate, which is the key to ensure the success and reliability of the MRS scanning of the prostate in patients with adequate preparation, accurate positioning specifications, reasonable parameter setting, good shimming effect and standard post-treatment.
