目的了解我院生殖中心不孕不育患者的超说明书用药的发生率及特点,为临床合理用药提供参考。方法采用回顾性分析,抽取我院2015年10月1日至12月31日生殖中心的所有门诊患者处方,记录患者的基本信息、临床诊断和用药信息等内容,筛选出临床诊断包含不孕或不育,年龄≤45 a的病例,根据药品说明书,由两名药师共同判断超说明书用药类型,并进行统计分析。结果共筛选出处方4 437张,含6 889条用药医嘱,其中含有超说明书用药的处方1 221张,占27.5%;有1689条超说明书用药医嘱,占24.5%;类型为超适应症1 045条(61.9%),超用法用量461条(27.3%),超给药途径22条(1.3%),超适应症同时超用法用量161条(9.5%)。超说明书用药使用频率高的药品主要为激素类和影响代谢类等药物。结论我院生殖中心不孕不育患者超说明书用药现象普遍,药事管理部门应规范其使用,临床医生和药师应收集循证医学证据,提高合理用药水平。 Objective To understand the incidence and characteristics of super instruction manual in infertility patients of reproductive center in our hospital and to provide reference for clinical rational drug use. Methods A retrospective analysis was performed to extract prescriptions from all outpatients in the reproductive center of our hospital from October 1, 2015 to December 31, 2015. The basic information, clinical diagnosis and medication information of the patients were recorded and clinically diagnosed with infertility or Infertility, age ≤ 45 a cases, according to the drug manual, two pharmacists to determine the type of super-description of the drug, and statistical analysis. Results A total of 4 437 prescriptions were obtained, of which 6 889 prescriptions were obtained. Among them, 1 221 were prescriptions containing super instructions, accounting for 27.5%; there were 1689 prescriptions for super instructions, accounting for 24.5%; type was super-indication 1 045 (61.9%), 461 (27.3%) cases of super-usage and 22 (1.3%) cases of super-administration, and 161 (9.5%) cases of super-indications. Super manual drug use high frequency of drugs mainly for hormones and metabolism and other drugs. Conclusion In our hospital reproductive center of infertility patients over the instructions of drug usage is widespread, pharmacy management should regulate its use, clinicians and pharmacists should collect evidence of evidence-based medicine and improve the level of rational drug use.
