为了探讨急性髓系白血病(AML)患者白血病细胞B7-H1基因表达的临床意义,本研究应用SYBR GreenⅠ实时定量RT-PCR法检测了40例初治AML患者骨髓单个核细胞(BMMNC)B7-H1 mRNA的表达情况,跟踪检测了10例复发患者BMMNC B7-H1 mRNA的表达情况,并对40例AML患者的临床特征与B7-H1基因表达水平进行相关性分析。结果表明:初治AML患者BMMNC B7-H1 mRNA表达阳性,但表达水平低于正常人。复发时B7-H1mRNA表达水平较初诊时明显升高。B7-H1基因表达水平与患者性别、年龄、外周血白细胞计数、骨髓中幼稚细胞比例、CD34抗原阳性与否均无相关性,但与治疗反应有显著相关性。诱导化疗后未获完全缓解(CR)的患者,其治疗前B7-H1 mRNA表达水平明显高于化疗后获得CR的患者。结论:初诊AML患者BMMNC B7-H1基因表达水平与患者在诱导化疗后是否获得缓解呈负相关。B7-H1基因表达水平有可能用于预测AML患者的预后。
In order to investigate the clinical significance of B7-H1 gene expression in leukemia cells of patients with acute myeloid leukemia (AML), SYBR Green Ⅰ real-time quantitative RT-PCR was used to detect the expression of B7-H1 in bone marrow mononuclear cells (BMMNC) The expression of B7-H1 mRNA was detected in 10 patients with recurrent disease. The clinical features of 40 AML patients and the expression of B7-H1 gene were analyzed. The results showed that: BMMNC B7-H1 mRNA expression in newly diagnosed AML patients was positive, but the expression level was lower than that in normal people. The recurrence of B7-H1mRNA expression was significantly higher than the first visit. There was no correlation between B7-H1 gene expression and patient’s sex, age, peripheral blood leukocyte count, proportion of immature cells in bone marrow, positive or negative of CD34 antigen, but significant correlation with therapeutic response. Patients who did not receive complete remission (CR) after induction chemotherapy had significantly higher B7-H1 mRNA expression than those who received CR after chemotherapy. Conclusion: The expression level of B7-H1 gene in BMMNC of newly diagnosed AML patients is negatively correlated with whether patients have been relieved after induction of chemotherapy. B7-H1 gene expression levels may be used to predict the prognosis of AML patients.
