我院1990—1995年,共收治胃癌176例,其中高龄(>70岁)胃癌患者28例,占同期胃癌手术患者的15.9％,现将治疗体会分析如下。 1 临床资料 1.1 一般资料 本组男18例,女10例,年龄70～85岁,平均74.2岁。男:女约1.8:1。其中胃底贲门癌5例,胃体部8例,胃角和胃窦部15例。 1.2 临床表现与病程 上腹部不适或腹痛11例;上消化道出血,黑便5例;幽门梗阻6例;消瘦12例;贫血11例;腹部扪及包块6例。从重视症状到就诊3个月之内7例;3～6月18例,6月～1年2例,1年以上1例。 1.3 TNM分期与病理 28例中,TNM分期Ⅰ期1例,Ⅱ期10例,Ⅲ期13例,Ⅳ期4例。癌细胞为高分化、中分化20例,低分化及未分化4例,印戒细胞癌2例,粘液腺癌2例。所有病例均有不同程度的淋巴结转移。
In our hospital from 1990 to 1995, a total of 176 cases of gastric cancer were treated, of which 28 cases were elderly (>70 years old) gastric cancer patients, which accounted for 15.9% of patients with gastric cancer during the same period. The treatment experience is now analyzed as follows. 1 clinical data 1.1 General Information The group of 18 males and 10 females, aged 70 to 85 years, mean 74.2 years old. Male: Female is about 1.8:1. Among them, there were 5 cases of cardial cancer, 8 cases of gastric body, and 15 cases of gastric antrum and gastric antrum. 1.2 Clinical manifestations and duration of disease Upper abdominal discomfort or abdominal pain in 11 cases; upper gastrointestinal bleeding, melena in 5 cases; pyloric obstruction in 6 cases; weight loss in 12 cases; anemia in 11 cases; abdominal hernia and mass in 6 cases. From the emphasis on symptoms to treatment within 3 months of 7 cases; 3 to 6 months of 18 cases, June to 1 year of 2 cases, more than 1 year. 1.3 TNM staging and pathology In 28 cases, TNM stage I was 1 case, stage II 10 cases, stage III 13 cases, stage IV 4 cases. Cancer cells were well differentiated and moderately differentiated in 20 cases, poorly differentiated and undifferentiated in 4 cases, signet ring cell carcinoma in 2 cases, and mucinous adenocarcinoma in 2 cases. All cases have different degrees of lymph node metastasis.
