目的 以跨理论模型为框架,探讨2型糖尿病患者在不同时期对胰岛素注射的认识感受.方法 采用质性研究中的现象学研究法,对8名处于不同时期的注射胰岛素的2型糖尿病患者进行非结构式访谈,并以Colaizzi现象学研究法分析资料.结果 按跨理论模型提炼出关于2型糖尿病患者3个不同时期注射胰岛素的认识感受的主题,3期分别为:①注射初期:接受较好,认可胰岛素,信任医生,惧怕并发症为主;②注射过程期:个人对注射的认识及家庭支持起到主要影响;③延续治疗期:接受情况好,但希望不注射最好.结论 2型糖尿病患者在不同时期接受胰岛素注射的认识感受是不同的,医护人员应有针对性地为其提供个性化心理疏导及健康教育,以保证胰岛素治疗的有效性、持续性.“,”Objective To investigate the cognition of patients with diabetes mellitus type 2 who receive insulin injections in different cognitive stagrs.Method In present study,phenomenological methodology of qualitative research was used,and 8 patients with diabetes mellitus type 2 who receive insulin injections in different stages were interviewed using semi-structural method,.The data was analyzed applying Colaizzi phenomenological research method.Results According to transtheoretical model,the process of insulin therapy could be divided into 3 different stages,and in each stage patients were with different cognition about insulin therapy.At the primary stage,the patients presented high acceptance of insulin injections,and believed in doctors,but afraid ofcomplications.During the second stage,the sustained process of insulin injection,the successful insulin therapy was mainly attributed to their own understanding of insulin injection and family support.The third stage,the prolonged injections stages,insulin injections were more acceptable,but no insulin treatment was expected by them.Conclusions The cognition and feelings of patients with diabetes mellitus type 2 differ much during the different stages of insulin therapy,so health workers should provide personalized interventions as well as health educationsto ensure the efficiency and continuity of insulin therapy.
