目的探讨血液透析患者心胸比与心功能及微炎症状态的关系。方法选取本院2015年6月-2016年6月进行维持性透析的患者80例作为研究对象,所有均采用测量患者心胸比,根据患者心胸比值分为>0.5的为观察组(40例),心胸比值分<0.5的为对照组(40例),对比2组患者心功能状态、微炎症水平。结果观察组患者左心室舒张期末内径为(73.8±4.3)mm,每搏输出量为(41.8±3.7)ml,射血分数为(49.4±5.3)%,B型尿钠肽为(678.4±11.3)ng/ml,与对照组患者比较,差异有统计学意义(P<0.05)。观察组患者IL-6、CRP水平明显高于对照组,差异有统计学意义(P<0.05)。Spearman相关性分析结果提示,心胸比与患者透析龄、e GFR、左心室舒张期末内径、每搏输出量、射血分数呈负相关(r值分别为-0.37、-0.36、-0.32、-0.33、-0.43,P<0.05),与B型尿钠肽呈正相关(r=0.36,P<0.05)。结论心胸比与患者炎症状态及新功能指标密切相关
Objective To investigate the relationship between cardiothoracic ratio and cardiac function and micro-inflammation in hemodialysis patients. Methods Totally 80 patients undergoing maintenance hemodialysis from June 2015 to June 2016 in our hospital were enrolled in this study. All of them were measured by measuring the cardiothoracic ratio of the patients, and the observation group (40 cases) Cardiothoracic ratio score <0.5 for the control group (40 cases), compared two groups of patients with cardiac function, micro-inflammatory levels. Results The end diastolic diameter of the left ventricle in the observation group was (73.8 ± 4.3) mm, the stroke volume was (41.8 ± 3.7) ml, the ejection fraction was (49.4 ± 5.3)%, and the type B natriuretic peptide was (678.4 ± 11.3) ) ng / ml, compared with the control group, the difference was statistically significant (P <0.05). The levels of IL-6 and CRP in the observation group were significantly higher than those in the control group (P <0.05). Spearman correlation analysis showed that the cardiothoracic ratio was negatively correlated with the dialysis age, eGFR, end-diastolic diameter of left ventricle, stroke volume and ejection fraction (r values were -0.37, -0.36, -0.32, -0.33 , -0.43, P <0.05), which was positively correlated with type B natriuretic peptide (r = 0.36, P <0.05). Conclusions Cardiothoracic ratio is closely related to the inflammatory status and new function indexes of patients
