目的 研究分析老年多原发癌患者首癌和次癌肿瘤组织Igκ和Igλ表达及特点。方法 2 1例长期随访的多原发癌病人 ,首癌为消化道癌的多原发癌者 2 0例 ,首癌与次癌均为消化道癌者 1 7例。应用鼠抗人κ轻链单克隆抗体和鼠抗人λ轻链抗体对肿瘤组织行免疫组化染色。结果 首癌或次癌有Igκ和 (或 )Igλ阳性表达者共 1 6例 ,占 76 2 %。统计学分析表明 ,多原发癌首癌与次癌肿瘤组织的Igκ、Igλ表达关系密切 ,具有明显的相关性(P <0 0 1 )。首癌与次癌Igκ、Igλ表达情况与多原发癌生存率统计分析提示 ,首癌与次癌Igλ同时阴性组与首癌Igλ阴性次癌Igλ阳性组比较 ,后者的次癌生存时间明显缩短 (P <0 0 5 )。结论 Ig样蛋白表达是多原发癌的癌变过程中较为普遍的现象 ,Igκ和Igλ表达在多原发癌的发生发展中可能起重要作用。与首癌、次癌Igλ表达均阴性的多原发癌病人比较 ,如在次癌肿瘤组织中出现Igλ表达阳性 ,预示此类多原发癌患者的生存时间短 ,预后差
Objective To study the expression and characteristics of Igκ and Igλ in primary and secondary cancerous tumors in elderly patients with multiple primary cancers. Twenty-one cases of multi-primary cancer patients with long-term follow-up, 20 cases of primary cancer with multiple primary cancers of digestive tract cancer, and 17 cases of primary and secondary cancers with digestive tract cancer. Tumor tissues were immunohistochemically stained with murine anti-human kappa light chain monoclonal antibody and murine anti-human lambda light chain antibody. Results The first cancer or sub-cancer with Igκ and / or Igλ positive expression of a total of 16 cases, accounting for 76 2%. Statistical analysis showed that there was a significant correlation between the expression of Igκ and Igλ in the primary carcinoma of primary tumors and in the secondary carcinoma tissues (P <0.01). The first cancer and sub-cancer Igκ, Igλexpression and multiple primary cancer statistical analysis showed that the first cancer and sub-cancer Igλnegative group and the first cancer Igλnegative sub-cancer Igλpositive group, the latter subcarcinoma survival time was significantly Shortened (P <0 05). Conclusion Ig-like protein expression is a common phenomenon in the carcinogenesis of multiple primary cancers. The expression of Igκ and Igλ may play an important role in the development of multiple primary cancers. Patients with multiple primary cancers who were negative for primary and secondary IgA expression were positive for Igλ expression in sub-cancerous tissues, indicating that these multiple primary cancers had short survival time and poor prognosis
