目的:总结国内外血氧水平依赖性功能磁共振(blood oxygenation level dependant functional magnetic resonance imaging,BOLD-fMRI)在胶质瘤患者治疗中的应用。方法:应用PubMed及CNKI期刊全文数据库检索系统,以“血氧水平依赖性功能磁共振、胶质瘤、治疗”为关键词,检索2008-01-2014-01的中英文相关文献。纳入标准:1)BOLD-fMRI在胶质瘤外科手术中的应用;2)BOLD-fMRI在胶质瘤放疗中的价值。符合分析的文献41篇。结果:BOLD-fMRI立体功能成像所获得结果与术中对大脑皮层进行电刺激所获得的结果具有较高的一致性,能够在术前能准确直观的显示运动及语言等功能区位置,评估手术的可行性。并且通过术中解剖和功能定位联合神经外科手术导航仪进行术中导航,在治疗过程中有效的避开重要的脑部功能区,提高了手术方案的准确性,缩短了手术时间并同时最大化切除胶质瘤。根据胶质瘤患者手术前后无创的BOLD-fMRI图像对比,能够全面评估手术效果及判断其预后,为术后放疗靶区的勾画及计划的制定提供重要的参考信息。结论:BOLD-fMRI是一种能够灵敏、准确评价脑局部区域血氧水平的磁共振功能影像技术,在胶质瘤手术治疗的选择、放疗计划的制定以及判断预后等方面具有潜在的临床意义。
Objective: To summarize the application of blood oxygenation level dependent cellular magnetic resonance imaging (BOLD-fMRI) in the treatment of glioma patients. Methods: Using PubMed and CNKI journals full text database retrieval system, the relevant data in Chinese and English from 2008-01-2014-01 were retrieved by using “blood oxygen level dependent functional magnetic resonance imaging, glioma and treatment” as the key words. Inclusion criteria: 1) the use of BOLD-fMRI in glioma surgery; and 2) the value of BOLD-fMRI in glioma radiotherapy. 41 articles in line with the analysis. Results: The results of BOLD-fMRI stereotactic imaging were consistent with the results of intraoperative cortical electrical stimulation. They could accurately and intuitively display the location of functional areas such as motor and language before surgery, Feasibility. And through intraoperative anatomy and functional positioning combined with neurosurgery navigator intraoperative navigation, in the treatment process to effectively avoid important brain functional areas, improve the accuracy of the surgical plan, shorten the operation time and maximize Excision of glioma. According to noninvasive imaging of BOLD-fMRI in patients with glioma before and after surgery, it is possible to evaluate the effect and prognosis of the surgery comprehensively and provide important reference information for the outline of postoperative radiotherapy target and the planning. Conclusion: BOLD-fMRI is a magnetic resonance imaging technique that can sensitively and accurately evaluate the blood oxygen level in the brain region. It has potential clinical significance in the selection of surgical treatment of glioma, the development of radiotherapy planning and prognosis.
