目的探讨C-反应蛋白(CRP)和癌胚抗原(CEA)对肺癌的诊断价值。方法选取2011年6月-2012年12月于温州医科大学附属第二医院就诊的65例肺癌患者作为肺癌组,同期收治的60例良性肺部疾病患者作为良性肺病组,同期行常规体检的健康人50例作为健康对照组。3组研究对象抽取静脉血,对血清中的CRP和CEA含量进行检测和分析。结果血清CRP和CEA水平肺癌组显著高于良性肺病组,良性肺病组显著高于健康对照组,差异具有统计学意义(P均<0.05);肺癌患者中血清CRP和CEA表达水平Ⅳ期显著高于Ⅱ期+Ⅲ期,Ⅱ期+Ⅲ期又显著高于Ⅰ期,差异均具有统计学意义(P<0.05);CRP与CEA联合检测诊断肺癌的敏感性为67.7%,特异性为95.4%,准确性为81.5%,均优于单个指标对肺癌的诊断。结论血清CRP和CEA水平对于肺癌的诊断有重要的价值,两者联合检测肺癌具有敏感性高和特异性好等优点,显著提高了肺癌诊断的准确率,值得临床推广。
Objective To investigate the diagnostic value of C-reactive protein (CRP) and carcinoembryonic antigen (CEA) in lung cancer. Methods Sixty-five patients with lung cancer who were treated at the Second Affiliated Hospital of Wenzhou Medical University from June 2011 to December 2012 were selected as the lung cancer group. Sixty patients with benign lung disease admitted in the same period were selected as the benign lung disease group and underwent routine physical examination 50 cases of people as a healthy control group. Three groups of subjects were drawn venous blood, serum levels of CRP and CEA were detected and analyzed. Results Serum levels of CRP and CEA in lung cancer group were significantly higher than those in benign lung disease group and benign lung disease group, and the difference was statistically significant (all P <0.05). The serum levels of CRP and CEA in patients with lung cancer were significantly higher than those in normal lung disease group (P <0.05). The sensitivity and specificity of combined detection of CRP and CEA in the diagnosis of lung cancer were 67.7% and 95.4%, respectively, which were significantly higher than those in stage Ⅰ, Ⅱ, Ⅲ, , Accuracy of 81.5%, better than a single indicator of lung cancer diagnosis. Conclusions Serum levels of CRP and CEA are of great value in the diagnosis of lung cancer. The combination of both of them has the advantages of high sensitivity and good specificity in detecting lung cancer, which significantly improves the diagnostic accuracy of lung cancer and deserves clinical promotion.
