目的探讨糖化血红蛋白和糖化血清蛋白联合检测在妊娠期糖尿病中的价值。方法选择2015年9月~2016年1月来我院行产前检查的孕妇169例作为研究对象,依照口服葡萄糖耐量试验筛查结果将其分为两组:正常妊娠组(107例)、妊娠期糖尿病组(62例)。分别测定两组孕妇的GSP、Hb Alc以及空腹血糖(FBG)并对检测结果进行对比分析。结果 1妊娠期糖尿病组孕妇血清FBG、Hb Alc、GSP分别为(4.39±0.69)m m o l/L、(5.7 4±0.7 7)%、(2.1 8±0.2 6)m m o l/L,均高于正常妊娠组(3.8 6±0.2 9)m m o l/L、(5.25±0.64)%、(2.1±0.12)mmol/L(P<0.01)。2妊娠期糖尿病组孕妇GSP、Hb Alc阳性检出率分别为67.74%、62.9%,均高于正常妊娠组47.76%、30.62%(P<0.01)。3妊娠期糖尿病组孕妇GSP、Hb Alc两者之间的阳性检出率(P>0.05)差异无显著性。结论 Hb A1c、GSP可作为妊娠期糖尿病筛查指标,但不够理想,其联合检测并不能提高筛查的阳性率。
Objective To investigate the value of combined detection of glycated hemoglobin and glycosylated serum protein in gestational diabetes mellitus. Methods From September 2015 to January 2016, 169 pregnant women who came to our hospital for prenatal care were selected as study subjects and divided into two groups according to oral glucose tolerance test screening results: normal pregnancy group (n = 107), pregnancy Diabetes group (62 cases). The GSP, Hb Alc and fasting blood glucose (FBG) were measured in two groups of pregnant women and the results were compared. Results The serum levels of FBG, Hb Alc and GSP in pregnant women with gestational diabetes mellitus were (4.39 ± 0.69) mmol / L, (5.7 4 ± 0.7 7)% and (2.1 8 ± 0.2 6) mmol / L, (3.8 ± 0.12) mmol / L, (5.25 ± 0.64)% and (2.1 ± 0.12) mmol / L respectively (P <0.01). The positive rates of GSP and Hb Alc in pregnant women with gestational diabetes mellitus were 67.74% and 62.9%, respectively, which were significantly higher than those in normal pregnancy (47.76% and 30.62%, P <0.01). The positive rate of GSP and Hb Alc in pregnant women with gestational diabetes mellitus (P> 0.05) showed no significant difference. Conclusion Hb A1c, GSP can be used as a screening index for gestational diabetes mellitus, but not ideal, the combined test does not increase the positive rate of screening.
