目的探讨男性尿道口尖锐湿疣患者人乳头瘤病毒(HPV)感染基因分型情况,以及不同基因型别与临床复发的关系,了解男性尿道口尖锐湿疣合并高危亚型HPV感染状况,为女性宫颈HPV感染预防提供新的思路。方法采用基因扩增结合基因芯片杂交技术检测了68例男性尿道口尖锐湿疣患者皮损中的HPV,CO2激光治疗后随访6个月,研究不同基因型别与临床复发的关系。结果 68例男性尿道口尖锐湿疣皮损中,67例检测出HPV阳性,占98.5%,共检出15种HPV基因亚型,以HPV11(46例,占68.7%)和HPV6(35例,占52.2%)为主,检出病例数从高到低依次为HPV11>HPV6>HPV16>HPV18>HPV35、52、58>HPV42、43、33、59。在67例HPV阳性患者中,低危型HPV感染44例,占65.7%,以HPV11和(或)HPV6为主;高低危型HPV感染23例,占34.3%,其中21例为高低危型HPV混合感染,2例为单纯高危型HPV感染。67例患者经CO2激光治疗后随访6个月,高低危型HPV感染组复发率为87.0%,单纯低危型HPV感染组复发率为63.6%,两者差异有统计学意义(P<0.05)。结论北京男性尿道口尖锐湿疣HPV基因型主要以低危型HPV6、11为主,高危型HPV感染大多合并低危型HPV感染,高低危型HPV感染组尿道口尖锐湿疣临床复发率明显高于单纯低危型HPV感染组。
Objective To investigate the genotyping of human papillomavirus (HPV) infection in male urethral condyloma acuminatum and the relationship between different genotypes and clinical recurrence, and to understand the status of HPV infection in male urethral condylomata and high risk subtype HPV. Infection prevention provides new ideas. Methods HPV DNA was detected in 68 patients with condylomata acuminatum by gene amplification and gene chip hybridization. The CO2 laser treatment was followed up for 6 months to study the relationship between different genotypes and clinical relapse. Results 68 cases of male urethral condyloma acuminatum lesions, 67 cases were detected HPV positive, accounting for 98.5%, a total of 15 kinds of HPV genotypes were detected, with HPV11 (46 cases, accounting for 68.7%) and HPV6 (35 cases, accounting for 52.2%). The highest number of cases was HPV11> HPV6> HPV16> HPV18> HPV35,52,58> HPV42,43,33,59. Among 67 HPV-positive patients, 44 cases were low-risk HPV infection, accounting for 65.7%, mainly HPV11 and / or HPV6; 23 cases were high-low-risk HPV infection (34.3%), of which 21 were high-low-risk HPV Mixed infection, 2 cases of high-risk HPV infection. Sixty-seven patients were followed up for 6 months after CO2 laser treatment. The recurrence rate was 87.0% in high-low risk HPV infection group and 63.6% in low-risk HPV infection group, with significant difference (P <0.05) . Conclusions The genotypes of HPV in male urethral buccal warts in Beijing are mainly low-risk HPV6,11, high-risk HPV infections are mostly associated with low-risk HPV infection, and the clinical relapse rate of urethral condylomata acuminatum is significantly higher than that of pure HPV Low-risk HPV infection group.
