目的探讨小切口白内障囊外摘除联合人工晶状体植入术治疗晶状体半脱位的可行性和技术要点。方法对24例(28眼)晶状体半脱位(范围≤120°)患者施行小切口白内障囊外摘除联合人工晶状体植入术。术中作自闭性巩膜隧道切口,环形撕囊或截囊,摘除白内障,将人工晶状体植入囊袋内。术后观察视力、人工晶体位置及并发症情况。结果 28眼中有25眼成功将人工晶状体植入囊袋内,2眼因晶状体囊膜不全行人工晶体单侧袢缝线固定术,1眼因囊膜缺失行双侧袢缝线固定术;所有术眼人工晶体稳定、居中;术后矫正视力均明显提高,与术前比较差异有统计学意义(P<0.01);无严重并发症。结论小切口白内障囊外摘除联合人工晶状体植入术治疗晶状体半脱位(≤120°)安全可行、疗效确切。
Objective To investigate the feasibility and technical points of small incision extracapsular cataract extraction combined with intraocular lens implantation for the treatment of subluxated lens. Methods 24 patients (28 eyes) with subluxated lens (range≤120 °) underwent small incision extracapsular cataract extraction combined with intraocular lens implantation. Intraoperative autosurgical scleral tunnel incision, capsulorhexis or capsulotomy, cataract removal, the intraocular lens implantation in the capsular bag. Postoperative visual acuity, intraocular lens position and complications. Results Twenty-five of the 28 eyes were successfully implanted with intraocular lens (IOL) in the capsular bag. Two patients underwent unilateral 袢 suture fixation with intra-lens capsular insufficiency and one with bilateral capsulotomy due to lack of capsule. All Intraocular lens was stable and centered. The postoperative corrected visual acuity was significantly higher than that before operation (P <0.01). No serious complications were found. Conclusion Small incision extracapsular cataract extraction combined with intraocular lens implantation for the treatment of subluxated lens (≤ 120 °) is safe and feasible.
