目的探讨胎盘及脐血中基质金属蛋白酶-1(matrix metalloproteinases,MMP-1)及其抑制因子-1(tissue Inhibitor of metalloproteinases-1,TIMP-1)表达与妊娠期高血压疾病(hypertensive disorder complicating pregnancy,HDCP)发生及疾病严重程度的相关性。方法选择2012年1月至2014年1月湖北医药学院附属太和医院产科收治的86例HDCP患者为研究组,其中妊娠期高血压组36例、轻度子痫前期组18例和重度子痫前期组32例,选取同期正常妊娠晚期孕妇60例为对照组。采用免疫组化(SP)法检测各组胎盘中MMP-1、TIMP-1的表达;酶联免疫吸附法(ELISA)检测脐血血清中MMP-1、TIMP-1的表达。结果胎盘中MMP-1在对照组、妊娠期高血压组、轻度子痫前期组、重度子痫前期组的阳性表达率分别为96.7%、77.8%、66.7%、23.1%;4组脐血血清中MMP-1的表达分别为(294.33±11.53)pg/m L、(247.78±20.32)pg/m L、(177.67±12.63)pg/m L、(124.68±15.41)pg/m L,4组间两两比较差异均有统计学意义(P<0.05)。4组胎盘和脐血血清中TIMP-1的表达差异无统计学意义(P>0.05)。结论 MMP-1与HDCP的发生和疾病严重程度之间存在相关性。
Objective To investigate the relationship between the expression of matrix metalloproteinases (MMP-1), tissue inhibitor of metalloproteinases-1 (TIMP-1) and hypertensive disorder complicating pregnancy , HDCP) and the severity of the disease. Methods Eighty-six HDCP patients admitted to Taihe Hospital Affiliated to Hubei Medical College from January 2012 to January 2014 were selected as study group, including 36 cases of gestational hypertension, 18 cases of mild preeclampsia and severe eclampsia 32 cases of pre-treatment group, select the same period of normal pregnancy, 60 pregnant women as the control group. The expression of MMP-1 and TIMP-1 in placenta of each group was detected by immunohistochemistry (SP). The expression of MMP-1 and TIMP-1 in umbilical cord serum was detected by enzyme-linked immunosorbent assay (ELISA) Results The positive rates of MMP-1 in the placenta were 96.7%, 77.8%, 66.7% and 23.1% in the control group, gestational hypertension group, mild preeclampsia group and severe preeclampsia group respectively. The cord blood The expressions of MMP-1 in serum were (294.33 ± 11.53) pg / m L, (247.78 ± 20.32) pg / m L, 177.67 ± 12.63 pg / m L, 124.68 ± 15.41 pg / m L, The differences between groups were statistically significant (P <0.05). There was no significant difference in the expression of TIMP-1 in the placenta and umbilical cord blood between the four groups (P> 0.05). Conclusion There is a correlation between the occurrence of MMP-1 and HDCP and the severity of disease.
