目的:探讨1型糖尿病(T1DM)患者心理痛苦的现状及其影响因素。方法:选取2019年8月至2020年1月于南京大学医学院附属鼓楼医院治疗的199例T1DM患者,当面向患者发放一般资料调查问卷、T1DM痛苦量表、医学应对方式问卷和领悟社会支持量表进行问卷调查。采用Pearson相关分析法分析T1DM患者心理痛苦得分与应对方式及社会支持的相关性。采用logistic多元线性回归分析法分析T1DM患者心理痛苦的影响因素。结果:199例T1DM患者中伴有糖尿病痛苦者173例,检出率为88.44%。T1DM患者心理痛苦总得分为(2.15±0.61)分。T1DM患者心理痛苦得分与面对、回避应对方式、家庭支持、朋友支持维度均呈负相关(n r=-0.867~-0.407,均n P<0.01),与屈服应对方式均呈正相关(n r=0.407~0.749,n P0.05)。多元线性回归分析结果显示:T1DM痛苦总分与共同居住人数(n β=-0.116),并发症(n β=0.091),严重低血糖事件(n β=0.061),采取面对、回避、屈服应对方式(n β=-0.118、-0.177、0.116)、家庭支持(n β=-0.363)和朋友支持得分(n β=-0.171)有关(均n P<0.05)。n 结论:T1DM患者的心理痛苦处于中等水平,其影响因素有居住状态、糖尿病并发症、近期严重低血糖事件、疾病应对方式和社会支持。“,”Objective:To evaluate current status and influencing factors of diabetes distress in patients with type 1 diabetes mellitus (T1DM).Methods:A total of 199 diabetes with T1DM treated in Nanjing Drum Tower Hospital Affiliated to Nanjing University Medical School from August, 2019 to January, 2020 were enrolled and investigated via general information questionnaire, T1DM Distress Scale (T1-DDS), Medical Coping Modes Questionnaire and Perceived Social Support Scale. The independent sample n t test and one-way analysis of variance (ANOVA) were used in the intergroup comparison. Pearson correlation analysis was used to analyze the correlation between psychological distress score and coping style, social support in T1DM patients. The multiple linear regression analysis was used to analyze the influencing factors of diabetes distress in T1DM patients.n Results:Among the 199 T1DM patients, 173 patients had diabetes distress, and the detection rate was 88.44%, the average score of T1-DDS was (2.15±0.61) points. The distress score of T1DM patients was negatively correlated with coping styles, avoidance coping styles, family support and friend support dimensions (n r=-0.867--0.407, n P<0.01); meanwhile, the distress score of T1DM patients positively correlated with yield coping style (n r=0.407-0.749, n P0.05). The multiple linear regression analysis showed that the total pain score of T1DM was correlated with the number of people living together (n β=-0.116), complications (n β=0.091), severe hypoglycemia events (n β=0.061), coping, avoidance, and yield coping methods (n β=-0.118, -0.177, 0.116), family support (n β=-0.363), and friend support scores (n β=-0.171, all n P<0.05).n Conclusions:The distress of T1DM patients is at a moderate level. The influencing factors include residence status, diabetic complications, recent severe hypoglycemia events, disease coping styles and social support.
