目的 :分析脑血管畸形 ( EVM)在磁共振成像 ( MRI)和磁共振共振血管成像 ( MRA)的表现 ,并评估MRI和 MRA在 EVM诊断方面的应用价值。方法 :1 8例经 MRI诊断为 EVM的患者 ,其中脑动静脉畸形 ( AVM)1 5例 ,隐匿性血管畸形 ( OVM) 3例 ,用 Siemens Vision0 .5 T超导磁共振系统作 MRA检查 ,采用 TOF-ti3d-multi-tra-tun序列。结果 :1 5例 AVM患者中 ,无出血的 AVM其特征性的 MRI表现为流空现象 ,均呈纡曲的低信号血管影。发生出血的 AVM患者中 ,因出血后处于不同阶段 ,MRI表现有所不同。 3例 OVM患者。 MRI无明显血管流空现象 ,均呈高信号 ,外周围绕低信号环。AVM在 MRA上可显示供血动脉、畸形血管团、引流静脉 ;OVM在 MRA上均未见异常。结论 :EVM在 MRI和 MRA上呈现特征性表现 ,可使绝大部分患者免予作创伤性的血管造影检查。
Objective: To analyze the performance of cerebrovascular malformation (EVM) in magnetic resonance imaging (MRI) and magnetic resonance resonance angiography (MRA) and to evaluate the value of MRI and MRA in the diagnosis of EVM. Methods: Eighteen MRI-diagnosed patients with EVM were enrolled in this study. Fifteen cases of AVM and three of occult vascular malformations (OVM) were examined by MRA in Siemens Vision0 .5 T superconducting MR system. TOF-ti3d-multi-tra-tun sequence was used. Results: Among 15 AVM patients, AVM without hemorrhage showed characteristic air-flow phenomenon and all showed low-signal blood vessels. In patients with AVM bleeding, due to different stages of bleeding, MRI performance is different. 3 cases of OVM patients. MRI showed no significant vascular flow phenomenon, were high signal, the outer periphery around the low signal ring. AVM in MRA can display the feeding artery, abnormal blood vessels, drainage veins; OVM no abnormalities in the MRA. CONCLUSIONS: EVM presents a characteristic manifestation on MRI and MRA that renders most patients exempt from invasive angiography.
